MISSION
The Nursing Program is offered to facilitate the holistic development
of the student into quality nursing graduates who are service-oriented and
dedicated individuals upholding the principle of love, service, and
commitment to mankind especially to their countrymen. 

VISION
PHINMA-AU recognizes the student as the central figure of all
educational efforts and programs. Hence, the College of Nursing, with it the
competency based, community and research-oriented curriculum, aims to
mobilize PHINMA-AU’s effort through dynamic, academic and administrative
mechanism of developing its nursing students into well rounded, productive
individuals of the province, the Region particular and of the nation of
general.
INTRODUCTION

Community Health Nursing relies heavily on the systemic process of


designing and delivering health services and nursing care to improve the health of
the entire community. It combines all the basic elements of professional, clinical
nursing with public health and community practice. Community health nursing is
essential particularly at this point in time because it maximizes the health status
of individuals, families, groups and the community through direct approach with
them. 
Community health nursing is based on identified community health nursing
needs and functions within total community health programs such as Expanded
Program on Immunization(EPI) this immunization primarily focuses on newborns
and infants (0-11 months) : Pregnant women for Tetanus toxoid Immunization to
prevent maternal and child infection and diseases like tuberculosis, diphtheria
and pertussis : Operation Timbang (OPT) which monitors the nutrition and health
status of Children aged (0-72 months) and the Family Planning Program (FPP) to
educate the people and for them to be cognizant of the Sexually Transmitted
Infection (STI). Other programs include Breast feeding, Environmental sanitation
and control of acute communicable diseases and so on. These Programs where
being implemented to eliminate and reduced the morbidity and mortality rate
from certain diseases and to promote healthy lifestyle and environmental health.
The College of Nursing in PHINMA Araullo University include the
Community Health Nursing in its curriculum to board the knowledge capabilities
of Nursing students to be able to become reliable and effective health care
provider as well as helping the government and the community people. Then as a
Nursing students must understand the concepts and models of the community
health nursing, the importance of health promotion and disease prevention and
health care planning.
CHN OBJECTIVES

GENERAL:
 At the end of the community posting, students are able to acquire adequate
knowledge, develop desirable attitude and skill(practice) in taking care of
individual, family and community with the effective utilization of available
resources in the community.

SPECIFIC:
 Understand the administrative set up and acquire adequate knowledge about the
functions of Health Care Center.
 To control and counteract environment
 Conduct community survey
 Do community diagnosis
 Go for home visit and give age and need based individual, family care and
community care
 Conduct health education programmers
 Participate in community health activities

General Profile

Barangay Calawagan is one of the 89 component barangay of


Cabanatuan City. It located in the North Eastern part of the City and is
classified as a Rural Barangay.
The Barangay covers a land area of 357.75 hectares. It has a total
inhabitants of 1,224 and a household 357 based on the latest census
conducted by PHINMA-AU Nursing Student.

Scope and Delimitation:


 The data presented on the table 1 primarily focused on the Civil Status of the residence
in Purok 1,2,3,4,5,6,7 and 8 of Barangay Calawagan, Cabanatuan City as being tallied to the
total population on the particular area on the dates of data gathering.
Analysis:
 Majority of the resident present on the Barangay on the Barangay in Purok 1,2,3,4,5,6,7
and 8 are Single accounting 465 or 37.99%; Child accounts to 33.42% and Married to
27.04%%; while Widow 1.14% and Widower on the accounts of 0.41% out of the total
population.
 Since one of the majority are married, health program such as family planning and good
parenting should be further make available for them.
 The gathered data further implied that the preponderance of the residents are Single
and they are considered to be matured enough to handle every situation they have the
ability to fully understand made wise decision and concerned for others that are
noticeable for them. Being Single is said to be a milestone in one’s life for the reason
that on this status in life the man and woman assumes more mature roles in the society.
Specially health issues or health deficits in the family are one of the challenges that a
married couple will eventually experienced. Married couples should always prepared
enough to face every situation that a life will bring.

Definition of Terms:
 Married- they are the resident present in the Purok 1,2,3,4,5,6,7 and 8 wherein man
and woman are unite by the sacred matrimony of marriage and living together in one roof.
Regardless if they have children or not.

 Child- they are the occupant in the area wherein the area wherein their age bracket
ranges from 0-14 years of age both males and females.

 Single- both man and woman lives in the area who is not yet married. Age bracket
ranges from 15 years old onwards.

 Widow (Female)- they are the dweller in the area wherein their husband or partner is
already deceased.

 Widower (Male)- they are the dweller in the area wherein their wife or partner is
already deceased.

Scope and Delimitation:

 The data presented above illustrated is the status of immunization of the children in
Purok 1,2,3,4,5,6,7 and 8 of Brgy. Calawagan Cabanatuan City. The data sources are
those children ages 0-11months.

Analysis:

 Based on our gathered data, there are20 infants ages 0-11 months are still ongoing
which means that there are more vaccination needed to complete immunization status.
 It indicates that those families who have children ages 0-11 months are actively
participating in effective implementation of the Expanded Program on Immunization.

Scope and Delimitation:
 The data presented above principally focused on the Family Structure of the families
surveyed in Barangay Calawagan, Cabanatuan City.

Analysis:
 Out of 277 families surveyed in the area, 142 families or 51.26% of them are
Nuclear families followed by 100 families or 36.10% are Extended Families and the
last is the Single Parent 35 or 12.64%.
 The data indicates that the families in the community are still upholding the
traditional approach in family preservation. It also show that in nuclear family it well
be  easier for the families ascertain sufficiently provide the need of its immediate
member in comparison with Extended Families certain conflicts with financial
communication and certain related issues may arise, which is exigent to resolve.
 Lastly open communication is an integral element in sustaining a family that is, it is
easy to achieve in Nuclear type of Family.

 The data presented above Principality focused on the Sized family Surveyed in Brgy
Calawagan, Cabanatuan City

Analysis:
 Majority or 50% of the families in the community are Medium  Size Families it means
that the need of each and every Member of the Family are easily provide to them,
conflict are least likely to rise in a Family with only members unlike with more Family
Members.

Scope and Delimitation:
 The data presented focused on the Highest Educational Attainment of each and every
occupier on the Purok 1, 2,3,4,5,6,7 and 8 on the Brgy. Calawagan, Cabanatuan City,
Data gathering was conducted on all Thursdays and Fridays of July 2019.

Analysis:
 The Frequency and Percentage Distribution on the Resident in Purok1, 2,3,4,5,6,7 and 8
in Brgy. Calawagan Cabanatuan City according to their Highest Educational Attainment is
Reflected above; it can be noticed from the result  that out of 1224 resident 335 or
27.37% of them are Secondary or High School Graduates which comprises the majority
of the residents followed by Elementary graduate and secondary Level who are 149 or
12.17% while Elementary Level rank number Three on Three list with a frequency and
percentage distribution of 148 or 12.09% respecting on the other hand there’s 30 or
2.45% resident with Non- formal Education that answering survey

Scope and Delimitation:

 The principal source of data are those residents in the purok 1,2,3,4,5,6,7 and 8 of the
Barangay Calawagan.

Analysis:
 The data shows that 858 or 70.10% of people ages 18-45 years old are Unemployed and
Self employed are greater than the employed which account to the 180 or 14.70%
residents and lastly the OFW’s account to 33 or 2.70%.

 The data presented above primarily focused on the Dialect commonly spoken by the
residents in Purok 1,2,3,4,5,6,7 and 8 of Brgy Calawagan Cabanatuan City as being
tallied to the total population on the area on the dates of data gathering. Data gathering
was conducted on all Thursdays and Friday July 2019

Analysis: 
 The data shows that out of 277 families surveyed in Purok 1,2,3,4,5,6,7 and 8 of Brgy
Calawagan 97.11% or 269 families are using Tagalog as their mode of communication,
on the other hand there are also 8 families or 2.89% are communicating using Ilocano as
their dialect.
 The data presented above implies that the communication is not a problem in the
community dissemination of health related information is trouble free for health
workers.

 The data presented above primarily focused on the religion of the residents of Brgy.
Calawagan, Cabanatuan City as being tallied to the total population on the area on the
dates of data gathering. Data gathering was conducted on all Thursdays and Fridays of
July(22) to August 2019.

Analysis
 Majority of the residents in the community are Roman Catholic Accounting to 92.15% of
1128 resident followed by Iglesia ni Cristo with frequency and percentage distribution of
55 and 4.49% , Born Again with 1.95%, Baptism with 1% and Methodist with 0.41%.

Scope and Delimitation


 The Data presented above primarily focused on the Housing Construction of the houses
in Brgy. Calawagan Cabanatuan City as being tallied to the total number houses in the
area.
Analysis
 Majority of the houses in Barangay are made of Concrete Materials which comprises the
highest percentage of housing accounting to 154 or 55.60%.

Definition of terms:
 Concrete houses-  type of houses wherein the floors wall and fences
are all made of concrete materials.
 Concrete or wooden houses- houses that are made of combination of
wood and concrete materials.
 All wood or wooden structure houses – type of house where the
materials used are woods.
 Makeshift or Barong-barong – Pertains to temporary structure houses
primary made up of scrap or ordinary raw materials.

Scope and Delimitation


 The Data presented above principally focused on the House Ownership of the Families
in the area, as being tallied to the total number surveyed families in the community on
the dates of data gathering. Data gathering was conducted on all Thursdays and Fridays
of July (22) to August.

Analysis
 The shows that out of 277 surveyed families in Brgy. Calawagan 231 of them or 83.39%
already owned their houses 17 houses are rented and 29 houses are rented free.
 Indicated that majority of the families already posses their own houses and thus they
are able to allocate more of their budget to other family needs, however those who are
renting their house have lesser opportunities of acquiring other need because a part of
their budget goes to paying the monthly fee.

 The data presented above principally focused on the Well ownership of the families in
the areas as being tallied to the total number of surveyed families who are Well as their
Source of Water.

Analysis:
 Majority of the families in the Barangay owned their well accounting total number to
192 or 69% while those who are sharing the well are 85 or 31%. Those families who
owned their Well can get water easily providing the necessary needs of each individual.

Scope and Delimitation:


 The data above principally focused on the Water Source Facilities of the families.
Analysis:
 There are 35.02% Families in the area using Pitcher pump as their water source of
water, 15.88% using Deep well and 27.44% are shallow well, there are also 12.27% using
the Mineral water and 9.39% using Electric Power pump.

 As the data illustrates it shows that there are still families utilizing conventional water
source facilities, these families need extensive health education in proper handling of
water. Insufficient knowledge and inappropriate practice in the handling of the water
from the source to the strorage in the house could contaminate drinking water

Scope and Delimitation:


 The data presented above principally focused on the Methods of Garbage Disposal
utilized by the families in Barangay Calawagan as being tallied to the total number of
surveyed household.

Analysis:
 Majority of the resident in the community practice Garbage Collecting as their means of
garbage disposal account to 66.06% followed by Composting accounting to 21.30%, there
are also 9.75% families who practicing burning and 2.89% families who are practicing Open
Dumping.

 The data shows that out of 89.17% families surveyed in purok 1,2,3,4,5,6,7 and 8 of
Barangay Calawagan owned their Toilet area, while 10.11% sharing toilet with the other
families and there is 0.72% does not have their own toilet facility

The data shows that out of 14.08% families surveyed in purok 1,2,3,4,5,6,7 and 8 of Barangay
Calawagan using Flush in excreta disposal, while 85.92% using De-Buhos excreta disposal.

Scope and Delimitation:


 The data shows the frequency and percentage distribution of MCRA acceptor and Non
Acceptor of the Family Planning Program in Purok 1,2,3,4,5,6,7 and 8 of Barangay
Calawagan Cabanatuan City. The Principal sources of data are all Married Couple under
Reproductive Age which is 15-49 years of age.

Analysis:
 The number of MCRA (Married Couple under Reproductive Age) acceptor is 45.89%
while the Non Acceptor accounting to 54.11%.

Strength
 Maximum people in the community has their own farm and wide
plantation.
 Barangay Hall and Church is available in community for people to go with.
 Elementary School for children is accessible in community with good
security.
 Availability of electricity and water supply.
 A business of majority people such as: Sari-Sari Store, Grocery Store, and
Vegetable & Fruits Store.
 An easy access of transportation.
 Effective system and hospitality cooperation is most influensive traits of the
Barangay Officials.

Weakness
 Majority of people in the community has not able to finish their education.
 Lack of knowledge for some people about proper hand washing to prevent
some illnesses experienced especially on children.
 Some people is still not participating in any activities or programs in Brgy.
Hall.

Opportunities
 Accessibility for Health Services such as free medications and
immunizations for people in community through the help of Barangay
Health Center and most nearby hospitals.
 The community has rich in farms and huge plantation is a big impact for
Agriculture.
 Monitoring the assistance by the Cabanatuan City Health Office.
 Highly accessible for all student to continue their studies until college with
offered scholarship in the city.
 Health care services from the Rural Health Unit through our medical
manpowers and the presence of student nurses’ and clinical instructors for
conducting health information including health education and promotion.
Threats
 The community is highly vulnerable in flooding through nearby river and
irrigation.
 A great number of unemployed people living in the community which is
result to poverty.
 A greater number of people that have Acute Respiratory Tract Infection
which affect their health condition.
