    NOTICE OF FINAL DEMAND

    Madame:

    This pertains to the apprehension conducted by the Illegal Connection and Investigation
    Unit (ICIU) against your water service connection last October 30, 2015, where it was found out
    that you have knowingly, willfully and maliciously committed an unauthorized re-opening of a
    closed water service connection, which act is penalized under Republic Act No. 8041 otherwise
    known as the “National Water Crisis Act of 1995."

    As a standard operating procedure of this Office, your case was referred to the
    Committee on Assessment, for the proper computation of the illegal connection penalty
    imposable against your account. Last November 06, 2015, the Committee came up with an
    Assessment, charging you the amount of One Hundred Twenty One Two Hundred Seventy Five
    Pesos (Php 121, 275.20), a copy of which is herein attached as Annex “A.”

    Please be informed that a Resolution dated February 13, 2014, has been issued by this
    Office, finding you liable to pay the said penalty. A copy of the resolution is attached as
    Annexes “B”

    In this regard, FINAL DEMAND is hereby made upon you to pay or settle the above-
    stated amount including your unpaid bills of Two Hundred Forty Six Pesos and 40/100 (Php
    246.40) or for a total amount of ONE HUNDRED TWENTY ONE THOUSAND FIVE HUNDRED
    TWENTY ONE PESOS AND 60/100 (Php 121,521.60), within SEVEN (7) days from receipt hereof.

    Please be informed that a Board Resolution has already been passed authorizing this
    Office to file a case against you for violation of the above-cited law, should you fail to pay or
    settle your obligation on or before the period given.

    It is hope that you will give this demand your preferential attention. Thank you very
    much.
