This agreement is made between KET IAN C. COTALES (hereafter known as the “Choir Master”) and ____________________________________________________ (hereafter known as the “member”) of Grade ___________ (year), Section _____________ informally agreed to work together on the different projects and events hosted by the OCSAT Chorale. Moreover, both parties agreed to work and support Project Himig: An Innovation to Hone Individual Musical Gifts as their income-generating project for School Year 2019-2020.
The Choir Master agrees to:
Provide the needs of the member upon the availability of funds;
Take care of the welfare of the member on the duration of his/her membership in the organization; Grade the member for the Music and Arts under the subject, MAPEH; Follow-up with other MAPEH teacher with regards on their academic standing.
The member agrees to:
Be in his/her behavior while member of the organization;
Attend practices/rehearsals imposed by the choir master religiously which includes being on-time unless on situations stipulated on Clause 4; Attend important events set by the organization; Submit explanation either through letter or personal appearance in case of absence and will be considered only in the following situation: (1) school-related activities and (2) near-death situations; Fully give consent to the choir master to decide on his/her academic standing in the event of non-compliance to these terms.
