prenatal check-up. Maria lives at 33 Bonifacio St. Barangay 2 Lucban, Quezon. Midwife Leah
noticed that she has Philhealth. The estimated cost of the maternity package in the facility is

a. Where is her plan to deliver? Who will attend her delivery? She wants to deliver at
birthing home at Barangay Abang. She prefers that whoever attends her delivery
is the one on duty at the birthing home.
b. Who will take her to the birthing home when she is in labor and who will take care
of her children when she goes to the hospital? She prefers that her husband,
named Jose Santos will be the one to accompany her to the birthing home. His
contact no. is 0917 742 4913. Her youngest sister Jocelyn dela Cruz will take care
of her children when she is confined in the birthing home.
