Domestic Violence Aggravated Assault

On 01/30/2021 I was dispatched on an Assault Investigation located at 1701 West North Temple
but that occurred at .

Upon arriving medical was on scene and talking to a female that was
crying and had blood all over his face and chest. Medical stated that she needed stitches in the cut
that was above her left eye and that she had a quarter sized hemotoma on the back of her head.

I used my flash light while medical was speaking to her and notices the left side of her face near he
jaw was swollen and had an quarter size abrasion on it. Along with a cut above her left eye that was
hard to tell how bad it was due to how bad it was bleeding.

I called for Crime Lab to take photos of her injuries.

While was getting medical attention I spoke to her Mother and

asked he what happened and she stated that boyfriend Vos, Terence(01/20/1990) beat
her up. stated that Vos also tackled her in November, beat her up and broke her leg. I asked
if reported it and she stated that she did not due to her thinking that Vos would kill her.

I spoke to and she was very upset and crying. I asked her what had happened and she
stated that Vos beat her up. stated that Vos became very agitated and started hitting her
in the face several times. stated that she tried to put her hands up to protect her face and
stated that her arms were currently sore. I asked her to see them and I did not see any injuries on
her hands or arms

Crime Lab arrived on scene and took photos of injury and left without incident..

I asked where this occurred and she stated that she was not sure of the address but that
it was near a church and at his mom's home. Officer Ferevski was able to find the address due to it
being listed as Vos residence in RMS. Officer Ferevski asked if it was on and she
stated that it was.

I asked if she was willing to fill out a Witness Statement and be a Victim and she stated
that she was.

in November and broke her leg but stated she was afraid to report it.

I gave a Witness Statement and she filled it out. I asked her if she was willing to sign a
Medical Release form for her hospital visit and she stated that she was. I asked her what hospital
she was going to and her mom stated that she was going to take her to Pioneer Valley Hospital
located at 3460 South Pioneer Parkway West Valley City. signed the Medical Release
form. I gave a Domestic Violence Blue Card with the case number on it and explained to
her who the Victim Advocates were and that they could help her with a protective order.
stated that she was worried that Vos was going to show up at her home and hurt them all. I told
them to call the police if he shows up and to not let Vos in.

I collected the Medical Release Form and Witness Statement from and she left the scene to
go to Pioneer Hospital.

I looked at Vos's history and he has a long history of violent crimes that include Aggravated Assault
on a Police Officer Aggravated Assaults, Robbery's Drive By Shooting Attempted Homicide and
several more crimes. Vos is also a well know gang member with Black Mafia Gangster Gang.

I had 2 more Officers attach to the call along with Sergeant Hall due to Vos's criminal history and we
went and checked

Upon arriving answered the door and let us come in and check for Vos
but he was not there. i noticed that there was what looked like a bullet hole that went through the
front of the house near the front door. Officer Adair checked the back yard to see if there were any
foot tracks in the snow and there was not. There was a Maroon Tahoe( in the driveway that was
car. I asked if Vos had a car and stated that Vos usually gets around in
her car or car.

Sgt. Hall stated that there was history with Vos at . Myself and
Officer Ferevski went to this residence and it was Vos's grandparents home. They stated that they
had not seen Vos in a couple of weeks and let us come in and see if he was there and he was not.

I contacted Vos's parole Officer, Officer Walker and he stated that Vos was usually at his mom's
home and that he has an Ankle Monitor on but that it was not working so he could not locate a
location for him.

I noticed the history of and as one of her charges so I contacted

Officer L. Johnson and asked her if she new Vos and she stated that she did and that he is possibly
hanging out with ) who has felony 2 warrants for Assault on a Police
Officer.

A possible Address for was Palmer Court. I contacted Palmer Court and they stated that
and Vos do not currently reside there but that has been seen there within the last two
weeks. I told her to call us if he shows back up and asked to call us and refer to this case number.

I contacted by phone due to her being pretty upset on seen to see if she had any leads as
to where Vos might be and she stated that she did not. I asked her what caused this assault and

she stated that they were our for some drinks with some friends and Vos accused her of looking at
other guys and started to be come agitated so they left. stated that when they got to his
mom's house that was still agitated and she was going to leave and Vos then started hitting her
several times.

I noticed in that she had a so I asked her if Vos was

Pimping with and she stated that he was not and that he was just her boyfriend. I asked
her if he was armed and she stated that she did not know.

I told if Vos showed up at her mom's house( where

she was living to call police and not to answer the door and she stated that she would.

I scanned the Witness Statement and Medical Release form in and sent them to Records Via email. I
placed the originals in the Records box at Pioneer Precinct.
