    To facilitate liaison and arrangement, the person-in-charge should complete this form and return it to the Program Section of Sai Kung
    Outdoor Recreation Centre, 21 Hong Kin Road, Tui Min Hoi Area, Sai Kung, N.T. by fax (fax no. : 2792 0203), by post or in person, 1
    month before the check-in date. If the Centre does not receive the completed form by the deadline, activities of the camp day will be
    arranged by the Centre. The Centre’s decision shall be final.
