    1) Clarisse Martin works at Dr. Goodhealth Family Clinic and knows Dr. Simon Aranas who also worked there. 2) Martin messaged Aranas on Viber after someone called about a home COVID test by the clinic. 3) The person said they got a test from the clinic's "Ronald Añonuevo" for lower than the clinic's price and Aranas' signature was on the medical certificate. 4) Aranas and Martin confirmed the clinic did not do a home test that day, they don't have an employee by that name, and the signature was not Aranas'.

    AFFIDAVIT

    I, CLARISSE P. MARTIN, of legal age, single and a resident of #30 Road 3 Doña Petra
    Tumana Marikina City, after having been duly sworn in accordance with law, hereby
    depose and say:

    1. I am a Customer Service Specialist of Dr. Goodhealth Family Clinic


    (“Clinic”) located at 444 JP Rizal St cor P. Burgos St., Sto. Niño, Marikina City.

    2. During the course of Dr. Simon Allen S. Aranas’ (“Dr. Aranas”)


    employment in the Clinic, I personally got to know him.

    3. On 2 August 2021, I messaged Dr. Aranas through the Viber Application,


    using the number, 09190690776 (previously Larry LM. Martin’s, my brother’s
    number), wherein I inquired on whether or not he knew a certain Ronald
    Añonuevo.1

    4. He answered that he does not and I told him that someone had called the
    Clinic earlier regarding our Home Services, specifically, the swab testing for
    COVID-19 detection.

    5. I said that the Home Service Fee for the RT-PCR Swab Test was pegged at
    P 3, 000, to the caller. The latter was then surprised to hear about the price.

    6. He stated that his relative was swabbed by Ronald Añonuevo, a


    purported employee of the Clinic, in Mandaluyong City for a much lower price.

    7. He further stated that the price was only P 1, 100 and that a certain Dr.
    Simon Allen S. Aranas, was the signatory to the Medical Certificate which
    contained the results of the Swab Test.

    8. Astounded to hear about this, I asked for a copy of the Medical


    Certificate.2 Upon receiving the same through email, I sent it to Dr. Aranas.

    9. I was intrigued due to the following: a) I personally knew that there were
    no Home Services done by anyone from our Clinic, including Dr. Aranas, on 1
    August 2021; b) or in Mandaluyong City on the aforementioned date; and c) we
    have no employee by the name of Ronald Añonuevo.

    Attached as Annex “B” is the Medical Certificate issued to Arian V. Datog dated 1 August 2021 which
    was sent to me through email.
    10. Dr. Aranas also informed me that the signature on the Medical Certificate
    was not his, in our Viber conversation thread. 3 The screenshots I have attached
    herein were taken from Dr. Aranas’ mobile phone but I confirm that such, show
    our Viber conversation which I repeatedly referred to in this Affidavit.

    11. I thereafter informed him that an internal investigation was already


    ongoing.

    12. I am executing this 2-page Affidavit to attest the truthfulness of the


    foregoing facts and for whatever purpose/s it may serve.
