UNDERGRADUATE QUALIFIER’S GUIDE

Congratulations for being one of the 2021 qualifiers for the DOST-SEI S&T Undergraduate
Scholarships administered by the Department of Science and Technology – Science
Education Institute (DOST-SEI).

As you jumpstart in your journey as a DOST-SEI scholar, this Qualifier Guide has been
prepared to provide you with step-by-step instructions as to what you need to know,
accomplish, and submit when you avail the scholarship.

Kindly familiarize yourself and take note of the important details indicated in this guide.

STEP 1. Signing of the Scholarship Agreement and Clearance for Travel Abroad

After the virtual orientation and once you decide to avail of your scholarship award, you will
be provided with the following documents:

1. Scholarship Agreement (SA)


2. Clearance for Travel Abroad (Addendum)

Below are steps that you need to do relative to the above-mentioned documents:
a. Print 4 copies of the Scholarship Agreement on an 8.5” x 13” bond paper.
b. Print 2 copies of the Clearance for Travel Abroad on an 8.5” x 13” bond paper.
c. Affix your and your parent’s/legal guardian’s signatures on each page of the SA and
Clearance for Travel Abroad on the spaces provided.
d. Mail the 4 copies of the signed Scholarship Agreement and the Clearance for Travel
Abroad to the DOST Regional Office together with the following:
• Photocopy of the parent/legal guardian's government-issued ID card (Visit this
link for the list of accepted IDs: https://grit.ph/valid-ids/)
• Photocopy of the Notarized Affidavit of Guardianship (if documents will be signed
by the legal guardian)

The mailed documents will be checked if completely and properly accomplished. You will be
notified through email regarding the result of the evaluation.

STEP 2. Opening of the Land Bank of the Philippines (LBP) Payroll Account and
Facilitation of Enrollment/Endorsement to University Coordinator or DOST Regional
Offices/DOST-SEI

The DOST Regional Office shall assess your accomplished SA and if it is found complete and
in order, the following documents will be issued to you:

1. Letter of Introduction (LOI)

As a DOST-SEI scholar, you will be required to open a LandBank Visa Debit Card
(LVDC) Account. Kindly present the LOI to the LBP branch nearest you to open your
payroll account.

Submit the pertinent details of your bank account to DOST Regional Office.

2. Letter for Enrollment or Endorsement Letter

If you are enrolling in a private university, you will be issued the Letter for Enrollment
(LE). Kindly present this letter to the University Registrar to facilitate your enrolment.

The LE shall inform the university that you are a DOST-SEI scholar and that your tuition
and other school fees up to a certain amount per semester/trimester/quarter term
indicated therein shall be billed to DOST Regional Office. The exceeding amount shall
be borne by you and your parents.

On the other hand, if you are enrolling in a university where there is a University
Coordinator or in a university outside of your home region, you will be issued an
Endorsement Letter to the former or the DOST Regional Office nearest your school.
Kindly present the endorsement letter to the University Coordinator or the Office of the
DOST Regional Director, respectively, before you enroll in your school.

STEP 3. Submission of Other Scholarship Requirements

Aside from the accomplished SA and the Clearance for Travel Abroad, you are also required
to submit to the DOST Regional Office No. 5, Rawis, Legazpi City of the following:

A. Clear photocopy of the Certificate of Enrolment/Registration for the First Semester of


AY 2021-2022, if you are already enrolled;
B. Original copy of the Official Receipt for tuition and other school fees paid for the First
Semester of AY 2021-2022 (if already enrolled), for processing of the reimbursement.

It is important that you check the completeness and accuracy of the documents that you
will submit to the DOST Regional Office to avoid delay in the processing of your financial
assistance. The financial assistance due you will not be processed if you fail to submit the
required documents.

indicate your year of award, scholarship program and concern (e.g., 2021
Undergraduate_Status of Stipend) on the subject line for easy traceability.
