    Declaration of Bovine Sourced Material

    RE: Products Derived from Bovine Sources

    Based on the information available to the Animal and Plant Health Inspection Service
    (APHIS – USDA), I hereby declare,

    1. The product is derived from the hides of beef cattle and has not been fed renderings from other
    cattle.

    2. The product was derived from bovines born and raised in the USA.

    3. The product is of USA origin only.

    4. The product is derived from bovine animals which have been subjected to ante and postmortem
    inspection under a veterinarian’s supervision and were certified apparently free of infectious or
    contagious diseases.

    5. Approved Standard Operation Procedures are used and followed at the time of the controlled Closed
    Herd cattle are harvested for hides.

    6. The abattoir used to obtain the source tissue is located exclusively in the United States and is
    subjected to USDA approval and inspection by the USDA inspector.

    7. Hides derived from the controlled Closed Herd cattle are used for the product. No other animal parts
    are used.

    8. The hides do not come in contact with other parts (i.e. spinal column, nervous system and brain
    cavity) of the animal during the slaughtering process.

    9. The product is derived from bovine collagen which in turn is derived from the hide which does not
    come from the head of the animal.

    10. The product has been prepared exclusively from parts of slaughtered animals, which are fit for

    human consumption in accordance with Community legislation , but are not intended for human
    consumption for commercial reasons.

    11. The product was produced by a process that ensured that unprocessed Category 3 material is
    subjected to a treatment involving washing, pH adjustment using acid or alkali followed by one or
    more rinses, filtration and extrusion, in order to kill pathogenic agents and this processing occurs in
    the USDA inspected facility (Advanced BioMatrix’s USDA Facility Certification CA-COL-0003).
