1. Assess for patient’s overall condition which include the height, weight and any presence of
dehydration, anxiety, dizziness, nausea and vomiting to determine the appropriate dosage of the
drug and nursing management.
2. Check if there is backflow of blood in the IV-line site and if it is free from pain, redness and swelling
and the cannula is not dislodged. To ensure that it is patent and ready for the treatment.
Planning
1. Check that prescription is written clearly, if the doctor has reviewed the laboratory test and if
necessary, clarify with the prescribing doctor. To ascertain whether the client fits the treatment
while also observing the rights of patient in drug administration.
2. Check if the patient has signed the consent form. This is done as respect for the right of the client in
making appropriate decision for his health.
3. Check out that the appropriate antiemetics have been prescribed and administered. To ensure that
all medical interventions are ordered by the attending physician and that the flow of the procedure
follows the institution’s guidelines.
4. Check whether the drugs to be administered include vesicants or non-vesicants. Never administer a
cytotoxic drug without knowing the information. This is to determine appropriate interventions and
considerations specially when administering vesicant drugs.
5. Check for the, the date of administration, name of the drug and infusion fluid, the dosage of the
drug, prescribed volume, and name of patient that are necessary in observing the rights of patient in
drug administration.
6. Gather all the equipment and supplies needed for the procedure such as, normal saline, syringes,
ppe, gloves, masks, goggles, antiemetic drug, alcohol swab, iv administration set, iv cannula, iv tray
with the prepared cytotoxic drug and sterile gauze.
IMPLEMENTATION
7. Wash hands to prevent spread of microorganism, don ppe to protect the nurse from the cytotoxic
drug.
8. Remove the chemotherapeutic drug from the bag and place in foil container to protect drug from
light exposure which may affect its potency.
9. Flush the IV line with normal saline to check for the patency of the iv line and to determine whether
the vein will accommodate the extra fluid flow and irritant drugs.
10. Infuse the compatible flush to IV access to promote continuous flow of the drug and to avoid
irritation during the administration. Ensure that infusion of compatible fluid is fast running.
11. Using the non-touch technique, swab access device to avoid contamination of self-from the drug.
12. Place the gauze under the side port and swab with alcohol swab to reduce the number of pathogens
introduced by the needle.
13. Connect the syringe with the drug to the access port using luer-lock syringe to prevent the leakage
or separation, which may occur due to pressure during administration resulting in spray and
contamination.
14. Administer the drug into the fast-running infusion using a steady positive technique to promote
continuous flow of the drug and to avoid irritation during administration.
15. Once the bolus has been administered, flush with compatible flushing solution to prevent the drug
interaction. To prevent leakage of drug from the puncture site, on removal of the device.
Evaluation
1. Ensure that the chemotherapy prescription has been sign with the date. Document the drug
administration and any adverse effects in the chart to maintain accurate records and to provide
point of reference in any point of concerns.
