LEARNING OBJECTIVES
At the end of the session all student should be able
to :
1. Define nursing process
2. Explain the purposes of the nursing process
3. Explain the components of nursing process
4. Describe the process of developing nursing
diagnoses, collaborative problems, goals and
expected outcomes
5. Identify the information gathering techniques
and assessment as a tools in nursing process

NIETA
LEARNING OBJECTIVES cont…

6. Identify the process of planning a nursing

intervention in treatment of patient
7. Demonstrate the implementation process in the
nursing care for patient.
8. Recognize the evaluation process in providing
nursing care for the patients
9. Apply effectively nursing process in
documentations and treatment of patient .
10. Explain the important of critical thinking in
implementation of nursing process.
NIETA
DEFINITION

Nursing process is a problem-solving

approach that enables the nurse to
provide care in an organized scientific
manner.

NIETA
DEFINITION

“A systematic approach that is used by nurses to

gather patient data, critically examine and
analyze the data, identify the patient’s
response to a health problem, design expected
outcomes, take appropriate action, and then
evaluate whether the action is effective”.
Potter & perry (2001)

MAIN PURPOSES OF NURSING
PROCESS

“ The nursing process helps nurses

plan the care for their patients ”

NIETA
PURPOSES OF NURSING PROCESS

 Provide high quality care to the patients.

 Provides a means of diagnosing and caring for
patients so that they can reach their optimal level
of health.
 Identify the patient’s needs, help to determine the
priorities and work with the patient to establish
goals.
 The nursing process uses observational and
logical thinking to formulate and fulfill the goals of
nursing.

NIETA
Characteristics of nursing process

• Interactive, purposeful and systematic (organized)

• patient centered
• Goal- directed (outcome focused)
• Within the legal scope of nursing
• Prioritizing the needs
• The steps are interrelated and dependent on the accuracy
of each of the preceding steps
• Its used to identify, diagnose and treat human response to
health and illness

NIETA
Advantages of Nursing Process
• Provide an organize framework for meeting the individual needs
(pt, fmly, community)
• Steps of nursing process focus the nurse’s attention on the
‘individual’ human response of a patient – resulting in a holistic
plan of care addressing the specific needs of the patient
• Provide an organized, systematic method of problem solving.
Its will minimize dangerous errors or omissions in caregiving and
ovoid time-consuming repetition in care and documentation
• Provide direction for nursing activities.
• Enhance the opportunity for a nurse to use their knowledge,
expertise ad intuition constructively and dynamically to increase
like hood of a successfully pt outcome.
• Determine the pace of activities
• Central to all NURSING ACTION
NIETA
Component Of Nursing Process
• Nursing process consists of 5 DYNAMIC and interrelated
phases :

1. Assessment

2. Nursing Diagnosis

3. Planning

4. Implementation

5. Evaluation

Definition of critical thinking
• “Critical thinking is a cognitive process that utilizes thinking that is
purposeful, insightful, reflective and goal directed to develop
conclusions, solutions and alternatives that are appropriate for
the given situation”.

• Cognitive process during which an individual reviews data and

considers potential explanations and outcomes before forming an
opinion or making a decision.

• “Critical thinking in nursing practice is a discipline specific,

reflective reasoning process that guides the nurse in generating,
implementing, and evaluating approaches for dealing with client
care and professional concerns. ”
NLN 2000
NIETA
CRITICAL THINKING
IN NURSING PROCESS

The nursing process uses observational

and logical thinking to formulate and
fulfill the goals of nursing.

NIETA
Critical Thinking and Nursing Judgment

 How do we make decisions?

 How do nurses make decisions about

patient care?

 What do we rely on to help us in decision

making?

NIETA
Critical Thinking and Nursing Judgment

 Process acquired through hard work, commitment,

and an active curiosity toward learning.

 Decision making is the skill that separates the

professional nurse from technical or ancillary staff
 Good problem solving skills.

 Not always a clear textbook answer.

 Nurse must learn to question, look at alternatives.

NIETA
How do nurse's accomplish this?

 Learns to be flexible in clinical decision making.

 Reflect on past experiences and previous

knowledge.

 Listen to others point of view.

 Identify the nature of the problem.

 Select the best solution for improving client’s

health.
NIETA
Critical Thinking in Nursing

 Essential to safe, competent, skillful nursing practice.

 Based on principles of nursing process and the scientific

method.

 Requires specific knowledge, skills, and experience.

 Guided by professional standards and ethic codes.

 Requires strategies that maximize potential and

compensate for problems.

 Constantly reevaluating, self-correcting, and striving to

improve.
