Sub:-west Godavari District -Covid Hospitals – Palakol - Sukhibhava Nuring Home , Palakol-
Detailed Report - Submitted –Regarding

08.06.2021 8:00 p.m.

The last discharge was occurred at 7.30 p.m. and the total inpatients became Zero by that
time. Then after ensuring physically that the total on bed patients were Zero in both in ICU
and General Wards (ICU-6 beds and General-14 beds are vacant) , I have reported the same
to the District Authorities and also the hospital management as already represented on
02/06/2021 and they again expressed their inability to serve any new patients in view of lack
of staff as some of their staff were affected with Covid. And also the same was intimated to
the District Authorities.

08.06.2021 11:45 p.m.

At around 11:45 p.m.,I received a call from Dr.Suresh Babu of Sukhibhava Nuring Home,
Palakol and asked me to come to the hospital immediately. Then at 11:49 p.m., I make a
phone call to Help Desk land line and enquired the Help Desk staff about any urgency and
they informed that some team has come from State Control Room.

09.06.2021 12:15 a.m.

I was there at the hospital by 12:15 a.m. and at the time I came to hospital ,supporting staff
is present at help desk. They told me that Sri B.Karthik , HDM 3 Rd shift along with State Team
went to wards for verification of the beds .

At 12:25 a.m., the HDM of Sukheebhava along with Sri Shanmukh (Senior HDM, CHC,
Palakol) and Sri Pawan (HDM , Sri Surya Nursing Home, Palakol) came to help desk and CI ,
Palakol garu also present.

The HDMs have informed me that as per the instructions of the District Covid Co-
ordinator , Sri Shanmukh ( Senior HDM ,CHC, Palakol) and Sri Pawan (HDM , Sri Surya
Nursing Home, Palakol) along with Sri Karthik, HDM, 3rd shift inspected the Covid wards in
Ground floor and 1st floor and found that no patients were present in Covid wards, i.e, 6
beds in the ICU ward and 14 beds in general wards are vacant.

They further informed that even though no patients were present in the Covid wards at
Ground floor and 1St floor, as proceeded to 2nd floor, they found two patients in 2 nd floor
which was meant for medical and nursing staff and the three HDM’S reported that

1.Sri R.RatnaRaju, who was told that he was suffering from loose motions and as a relative
of Sri Vishak, trainee Doctor, he was staying in the room under first aid.

And 2.Smt.V.Sesha Bharathi Lakshmi is a Psychiatric patient.

CI , Palakol garu also told that only these two patients were present.
Later I have inspected the Ground floor and 1 st floor and found no patients in
Covid wards and later went to 2 nd floor along with HDM and two patients were found in 2 nd
floor meant for medical and nursing staff.

I have personally enquired Sri. R.Ratna Raju regarding his illness and he told
that he is suffering from loose motions due to post covid complications after completion of
home isolation and he further informed that as a teacher of Sri Vishak , trainee doctor, he
came directly to 2nd floor which was meant for medical and nursing staff and under first aid
without the notice of the Help Desk.

The 2 nd patient Smt. V.Sesha Bharathi Lakshmi was not responded even after
asking for several times and as she was woman and also as the time was midnight I enquired
Sri J.L.N.Nagendra Kumar who was a relative of patient, he informed that the patient has
been suffering from mental disturbance in post Covid period and told that came at 10.30
p.m. and previous night also came and went with out the notice of the help desk.

Then I came down and immediately briefed the proceedings to Sri M.Jaswanth, District
Covid co-ordinator.

I , the Nodal Officer nor the Help Desk Manager are completely unaware of admission of
these two patients in to the 2 nd floor rooms ( previously the hospital management informed
that 2nd floor was meant for Medical and Nursing staff ) .
