AFFIDAVIT OF CANCELLATION AND UNDERTAKING
I, MERELENE E. CANUZA, of legal age, Filipino, married and a resident of

Mainit, Nabunturan, Davao de Oro Province, Philippines, after having been sworn to in
accordance with law do hereby depose and states:
1. That I am the same person whose personal circumstances above indicated;

2. That I am the lawful and registered owner of a parcel of land described as Lot 4808-
B, Psd-11-064182 situated at Brgy. Mainit, Nabunturan, Compostela Valley (now
Davao de Oro) containing an area of SEVENTEEN THOUSAND FOUR HUNDRED
TWENTY FIVE (17, 425) square meters, more or less;
3. That a portion of the above-mentioned parcel of land was purchased by Mr. & Mrs.
JULIET SOTTO with P.S No. Psd-11-148142;
4. That it is my desire to have approved plan with P.S No. Psd-11-148142 in the name
of Mr. & Mrs. JULIET SOTTO be cancelled due to the fact that there is an existing
approved survey of TEN (10) persons who also purchased a portion of lot from me and
to change the manner of subdivision as shown with the herein attached subdivision
plan;
5. That the Sepia Copy of the aforesaid subdivision survey was lost and/or misplaced in
my possession and the same could no longer be found and/or located;
6. That diligent effort exerted to locate and/or recover the same but the same was
proved futile and useless;
7. That I hereby declare that there is no ongoing transactions with any
person/buyer or with the Register of Deeds, and that I assume full responsibility
in the cancellation of the above approved survey and that the office of the Land
Management Services DENR and Bureau of Land are not liable from any future
lawsuit that may arise in connection with this undertaking;
8. That I am executing this instrument in order to attest the truth and veracity of the
foregoing facts and for whatever legal purpose this may serve best.

AFFIDAVIT OF REVIVAL OF PSD-118209-125603
I, MERELENE E. CANUZA, of legal age, Filipino, married and a resident of

Mainit, Nabunturan, Davao de Oro Province, Philippines, after having been sworn to in
accordance with law do hereby depose and states:
1. That I am the same person whose personal circumstances above indicated;

2. That I am the lawful and registered owner of a parcel of land described as Lot
4808-B, Psd-11-064182 situated at Brgy. Mainit, Nabunturan, Compostela Valley
(now Davao de Oro) containing an area of SEVENTEEN THOUSAND FOUR
HUNDRED TWENTY FIVE (17, 425) square meters, more or less;
3. That the above parcel of land had been subdivided and had been approved as
Subdivision Survey No. Psd-118209-125603;
4. However, said subdivision survey Psd-118209-125603 had been cancelled to
pave way for approval of the new subdivision executed by Geodetic Engineer
Dalia C. Desella identified as Subdivision Survey No. Psd-11-148142;
5. That on October 18, 2021, undersigned owner executed an Affidavit of
Cancellation with Undertaking to cancel Survey No. Psd-11-148142;
6. That due to the execution of the foregoing instrument, it is my desire to revive
Psd-118209-125603;
7. That I hereby declare that there is no ongoing transactions with any
person/buyer or with the Register of Deeds, and that I assume full responsibility in
the cancellation of the above approved survey and that the office of the Land
Management Services DENR and Bureau of Land are not liable from any future
lawsuit that may arise in connection with this undertaking;
8. That I am executing this affidavit to state under oath the aforementioned facts
and to apprise the proper authorities thereof.
