INCIDENT REPORT
INCIDENT LOCATION DETAILS:

SCOPE:
This report describes the PETROJET incident and corrective action plan for the below described incident in line
with the findings of the site visit by PETROJET and CPMD safety/Project team, to avoid future recurrence of
such incident, to ensure the safety of the Saudi Aramco premises & properties, and to safe project operations.
There was no injury as a result of this incident but the incident resulted in damage/cut of two overhead electrical
lines.

INCIDENT DESCRIPTION:
The dump truck driver was transferring construction waste from ABGOSP-2 to the Temporary dumping area
outside Plant fence. While offloading construction waste, the driver raised the dump body and hit two overhead
electrical cables and damaged/cut them.

IMMEDIATE ACTIONS TAKEN:


1. The Driver switched off the vehicle engine and jumped out vehicle cabin to avoid electrical shock.
2. The driver immediately reported the incident to site engineer.
3. Site engineer immediately inform PETROJET HSE manager and SA permit issuer and isolated the area.
4. HSE Manager reported the incident to PETROJET Project manager.
5. The PETROJET HSE and Project manager immediately reported the incident to CPMD and activated
incident action plan to mitigate any further deuteriation of the situation.
6. PETROJET team immediately mobilized to the incident location to take necessary corrective actions and to
assist the POD and operation if needed.
7. PETROJET team assisted the Saudi Aramco POD and remained on the site until the electrical cable was
completely restored.

CAUSES OF THE INCIDENT:


1. The mentioned offloading activity carried out without banksman/flagman to guide the driver about blind
spots. The flagman left the driver alone after the security gate and return back inside the plant without
fulfilling his job duty to guide the driver till completing the job.
2. When the driver went to offload the construction waste in Temporary dumping area, he found the area
congested and he tried to offload the construction waste in nearby unsafe area without realizing that the
presence of overhead electrical lines.

PREVENTIVE ACTIONS TAKEN:


1. PETROJET project team instructed all the site engineers and supervisor to directly transport any waste to
ABQAIQ dumping area instead of dumping it in temporary dumping area.
2. PETROJET Project team instructed all the drivers not to drive the vehicle without banksman/flagman in
Saudi Aramco premises, restricted areas and Aramco corridors.
3. PETROJET project team advised to conduct BANKSMAN refresh training to avoid such incident to occur
again.

DISCIPLINARY ACTIONS TO BE TAKEN:
1. PETROJET project management terminated the services of Dump truck driver, Mr. Rafiullah
Mohammad and Flagman Mr. Gulwahab due to the negligence to their job responsibilities.
