TO ALL JUDICIAL DISPUTE RESOLUTION (JDR) - TRAINED


JUDGES OF THE FIRST AND SECOND LEVEL COURTS

RE: SUBMISSION OF MONTHLY JDR STATISTICAL REPORTS


TO THE PHILIPPINE MEDIATION CENTER (PMC), COURT
MANAGEMENT OFFICE (CMO), OFFICE OF THE COURT
ADMINISTRATOR (OCA)

In view of the transfer of the Philippine Mediation Center to the Court


Management Office, Office of the Court Administrator pursuant to the Supreme
Court En Bane Resolution in A.M. No. 21-08-01-SC-PHILJA dated October 5,
2021, all concerned JDR-trained Judges are hereby DIRECTED to submit
their monthly JDR statistical reports via e-mail to PMC-CMO, OCA
(pmeo.oea@judiciary.gov.ph), every 10th day of the following month, using the
attached PMC Form Nos. 12,21, and 22.

AGREEMENT TO MEDIATE

The undersigned appellant/s and appellees hereby declare the following:

That they agree with the determination of the Judge of this Court that there is still a
possibility of settlement between/among them;

That the benefits and advantages of Judicial Dispute Resolution on Appeal (JDRA) was
fully explained to them by the said Judge;

That after carefully considering such benefits and advantages, they agree of their own free
will to have their case be mediated for possible settlement of their dispute by the Judge through
JDRA;

That the parties understand that in the event JDRA fails, the appeal shall be decided by the
same Judge within the prescribed period; and,

That by virtue of this agreement, they shall be barred from seeking the inhibition of the
JDRA Judge for whatever reason.
