    AFFIDAVIT OF LOSS

    I, KIM B. LISONDRA, of legal age, single, Filipino and resident of 544


    Purok 6 Sucad, Apalit, Pampanga, after having been duly sworn to in
    accordance with law do hereby depose and say:

    1. That, on December 7, 2019, I took a TESDA Competency Test in


    Baliuag, Bulacan and as such, I was issued a CLAIM STUB for the
    release of my Certificate of Competency Assessment Result
    Summary (CARS) NC II in Events Management Services;

    2. That unfortunately, however, said CLAIM STUB was lost and/or


    misplaced for unknown reason;

    3. That I exerted efforts to locate or recover the same but proved in vain
    and to no avail;

    4. That I am executing this affidavit to attest to the truthfulness of the


    foregoing statement for the purpose of requesting the TESDA to
    release my Certificate of Competency Assessment Result Summary NC
    II in Events Management Services despite the loss of said CLAIM
    STUB and for all legal intents and purposes.

    IN WITNESS WHEREOF, I have hereto affixed my signature this 19 th day


    of December 2019, hereat Malolos City, Bulacan.

    #33B St. Ignatius Village, Mabolo, Malolos City, Bulacan, after having been
    duly sworn to in accordance with law do hereby depose and say:

    1. That, on August 20, 2018, I took a TESDA Competency Test in the


    College of Our Lady of Mercy, Baliuag, Bulacan and as such, I was
    issued a Certificate of Competency Assessment Result Summary
    (CARS) NC II in Massage Therapist;

    2. That unfortunately, however, said CERTIFICATE was lost and/or


    misplaced for unknown reason;

    3. That I exerted efforts to locate or recover the same but proved in vain
    and to no avail;

    4. That this affidavit is executed for the purpose of informing authorities


    concerned about the loss of the above-mentioned identification cards
    and documents, and further request them to issue in my favor copy of
    the lost one.
