A. Her name was recorded as ADELA A. CULITOY when in truth it

should be ADELA A. COLITOY;

B. Her date of birth was recorder as JANUARY 20, 1962 when in

truth and in fact, it should be JANUARY 29, 1963;

C. The name of her father appearing thereon was incorrectly written

as DIONISIO CULITOY, when in truth and in fact it should be
DIONESIO COLITOY; and

D. The name of her mother appearing thereon was also incorrectly

written as BIENVINIDA ALMOQUIERA, when in truth and in fact,
it should be BIENVENIDA ALMOQUERA.
