As the inviting person of the visa applicant (hereinafter referred to simply as “the Applicant”), I hereby agree to
the following items:
a) Write an invitation letter, secure an accommodation facility for the Applicant (only when home quarantine is
required), take measures such as preventing COVID-19 transmission, as well as measures in the event of a
symptomatic or positive case.
b) If any change occurs in the invitation details at the time of the visa application, ensure that the Applicant
reapplies before entering Japan.
c) In the event that the Applicant becomes symptomatic or tests positive for COVID-19, obtain information
beforehand such as the location or consultation hours of a medical institution near the accommodation facility.
d) Provide the Applicant with necessary information regarding Japan’s border measures, including a COVID-19
test at the time of entry and home quarantine rules through relevant websites or other materials.
e) If the Applicant is able to prepare a smartphone before entry into Japan, instruct him/her to (1) install MySOS
(the Health and Location Monitoring App for Overseas Entrants), (2) input information such as a COVID-19
test certificate and to complete screening beforehand as much as possible, (3) use Visit Japan Web (a web
service which allows the Applicant to go through procedures such as quarantine, immigration, customs at the
time of entry). If unable to do so, instruct the Applicant to rent a smartphone at the airport at the time of entry
(only when a home quarantine is required).
f) Secure an accommodation facility for the Applicant and provide information on transportation options to
ensure that the Applicant can move to the facility without fail (only when home quarantine is required).
g) Confirm the Applicant’s health condition every day by phone, e-mail, etc. (only when home quarantine is
required and excluding the case of required home quarantine at an accommodation facility designated by the
quarantine station).
In addition, cooperate on corrections and investigation with relevant authorities when contacted by them
regarding any violation of the pledge contents by the Applicant (violation of the Applicant’s pledge at the time
of entry), such as the Applicant not staying at the designated accommodation facility.
h) If the Applicant wishes to shorten the home quarantine period, arrange a COVID-19 test which meets the
requirements (only when home quarantine is required).
i) Promptly seek medical attention if the Applicant becomes symptomatic, positive or ill.
In addition, follow the instructions given by authorities, such as a public health center.
j) Ensure that the Applicant takes necessary measures to prevent infection accordingly, (1) wear a non-woven
mask, (2) thoroughly disinfect hands, and (3) avoid the “Three Cs” (Closed spaces, Crowded places, Close-
contact settings).
k) Comply with the laws and regulations related to the border measures systems, such as the Quarantine Act, the
Act on the Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases, and the
Immigration Control and Refugee Recognition Act.
l) Confirm that the purpose of the Applicant’s visit is not for tourism.
m) If any part of the abovementioned pledge is violated or if the Applicant violates the contents of the written
pledge that he/she submits at quarantine at the time of entry into Japan (including cases of misrepresentation
in either case), from the viewpoint of preventing the spread of COVID-19 infection, the name of the inviting
person may be made public, and the visa application of a person invited by the inviting person may not be
accepted thereafter.
