Subject :- Hiring of Dozer D50/A-15 for road work from pulsit to dhankuni Chanage No :-
623+00 to 631+500, NHAI - Bharatmala road project, Kolkata West Bengal.

Dear Sir,

With reference to the discussion, we had with you, we are pleased to issue work order for the subject work
with the rate and terms & conditions as mentioned: -

Detail Terms & Conditions: -

1. The Supplier has visited the site before quoting the rates and the site difficulties has been taken into
account while working out rate.

2. The rate is inclusive of all taxes but excluding the GST. The TDS shall be deducted as per the govt. rules.

3. The contractor shall not be permitted to demobilize/ remove the machine without the written consent of the
company along with the necessary gate pass.

4. The tentative period of hiring is 3 months and the likely date of start will be 06.01.23 However, the machine
hired from Jan Month 2023 and it shall be hired till the work completes the actual date of start shall be reckoned
from the date of work start at site. The Both side shifting charges of machine will be borne by us on actual basis.
If the requirement of Machine is less than Three month, one side shifting charges shall be borne by us on actual
basis if machine used more than the three month one side transportation from supplier scope.

5. The contractor shall maintain the Machine in good condition. He will submit all the necessary documents as per
the requirement of principal client and government authorities. The various lubricant, oil, spare parts, etc. shall be
arranged by the contractor to maintain the machine in good working condition.
6. Machine shall be engaged at site. shall be fitted with required safety features like three-point contact looking
glass, reverse buzzer, reverse light, working wind screen wipers, scotch block (manual wheel blocking system),
fire extinguisher in driver’s cabin, Hydraulic lift sensor and working head and tail lights.

7. Nobody is allowed to travel in machine other than

driver and his helper.

8. If your operator is caught stealing diesel from the crane, then his payment will be debited from your R A bill and
the crane can be removed from site with immediate effect as per instruction of NAP Officials.
9. The Machine may be shifted at other site as per the requirement of the company. The working hours shall be 260
Hr Each shift in a month, 6000/- Per Month Fooding allowance to be paid in cash onsite to the operator.

10. One operator and one helper will be there at site on each shift for regular operation and maintenance of
Machine.

11. The staff of the Machine shall be fully trained especially for the work the operator and helper shall be available at
work site at every-time, so that the same may be used as per the requirement.

12. The supplier will be fully responsible to strictly observe all the safety procedures and shall be solely responsible
for any miss happening or accident.

13. The necessary comprehensive insurance of the Machine along with staff engaged at site shall be done by the
supplier before the commencement of work.

14. If the company is at any time dissatisfied with the performance of the contractor the said contract shall be
terminated by the company without assigning any reason to the supplier.

15. The payment of shifting charges along with fifteen days advance will be released immediately after mobilization
of machine at site and the fifteen days advance will be released after 10 days of working. The total advance of
one month will be recovered in three subsequent bills in equal installments.

16. The monthly bill shall be submitted in the 1st week of every month along with the log book duly signed by your
representative and the official of the company and other documents as per the instructions of the Project Engineer
and payment of the same shall be made before the 15th day of the next calendar month.

17. The accommodation of operator and helper will be provided by us free of cost.

18. Parking space to park the Machine shall be arranged by us. However, the responsibility of watch & ward of
Machine shall be yours.

19. Diesel will be provided by the company free of cost. However, in case of excess consumption, recovery will be
done from running bills

20. Weekly maintenance day shall be decided by the Project Engineer, if there is a machine breakdown, and labor is
engaged with some other work, then when the machine work starts then the machine count shall start. else
breakdown will count

21. The rate is for 260 hours of working in a month limited to 30 days of working Single double shift & weekly off
as per direction of Project Engineer for maintenance of Machine. In case of break down, the recovery shall be
made on prorate-basis. If the number of days of breakdown exceeds more than two in a month, the contract shall
be terminated. / Rental Agency Required two days holiday’s for maintain good we can give him every
Sunday half day.

22. You are requested to sign and return second copy of this order as token of your acceptance.
