ACADEMIC POLICIES
AND GUIDELINES
AY 2022-2023

I. Weekly Learning Guides


a. Strictly follow the latest template including the header, footer, dates, and signatories.
b. As a response to the ISO and PAASCU recommendations, include the word Pre-assessment
where it is applicable under the Preparation/Motivation part of your WLG and the delivery of
instruction.
Example:
As a pre-assessment activity, the teacher will present a trivia.
c. Make sure to sign your WLGs.
d. Proofread your LPs before printing them.
e. After printing, fasten your WLGs together using the spiral binding machine at the Subject
Coordinators’ Office.
f. Submission of WLGs is every Monday at 4:00 p.m. For late submission, always attach a letter
of explanation addressed to the Subject Coordinator.
g. Integrate the corrections made by the Subject Coordinator in your succeeding WLGs. However,
for corrections in the learning objectives, the corrected version must be presented to the
students or should be written on the board.
h. Get your checked WLGs from the Subject Coordinators’ Office
k. Make it a habit to bring your checked WLGs in your classes.

II. Meetings
a. Meetings for subject areas and other units are scheduled accordingly. Please keep updated of
the calendar of activities.
b. Observe protocols during the meetings.
c. Attend all meetings punctually. Bringing of meeting notebooks is a must.
d. Participate actively in all meetings. Using of cellular phones and other gadgets and frequent
going out of the meeting venue is not allowed.

III. Submission of Documents

A. Class Records, Grade Sheets and Grade Analysis


a.1 Secure copies of Class Record sheets from your subject coordinators.
a.2 Formative and summative assessments must be duly recorded in the class records
sheets.
a.3 Class records will be checked on the following schedules:
a.3.1 Three days after the Mastery Test
a.3.2 Day 1 of the Periodic Examination
a.4 Submit Grade Sheets together with the checked Class Records.
a.5 Follow the schedule of submission of Grade Sheets and Grade Analysis indicated in the
Academic Calendar of Activities.
a.6 Submit the Grade Analyses, Deliberated Grade Sheets, and Final Grades to the Subject
Coordinators. The Subject Coordinators will forward the Deliberated Grade Sheets and Final
Grades to the Assistant Principal.

B. TOS/Test Drafts
b.1 Prepare Test Drafts only (for Mastery/Monthly Assessment Test) and TOS with Test Drafts
(for Periodic Examinations). Observe the agreed SOPs in test construction.
b.2 For Periodic Examinations, organize the coverage of the topics for the quarter and prepare
the matrix of the Table of Specification.
b.3 Submit the TOS to the subject coordinator for checking of the distribution of
Test items on Bloom’s Taxonomy of Educational Objectives against competencies
required for the grading period.
b.4 .Prepare the manuscript of the initial draft of the test upon the approval of the TOS
following the criteria:
b.4.1 topics to be assessed
b.4.2 formulation of the test items (grammar, spelling ,sentence construction)
b.4.3 kind of questions to be formulated for the grade level to include test directions and
instructions
b.4.4 font size and style.
b.5 Review the contents of the test draft and verify the coverage ensuring that the
desired goal of the subject using the Instructional Guide, Learning Plan and other
references are followed.
b.6 Examine the test draft to ensure that important topics being assessed are covered.
b.7 Prepare the final copy of the test draft for submission.
b.8 Submit the TOS/Test Drafts first to the Assistant Principal for recording. For late
submission, always attach a Letter of Explanation. The Assistant Principal will forward the
submitted TOS/Test Drafts to the Subject Coordinators for checking.
b.9 Edit the checked Test Drafts for final copy
b.10 Submit the Test Drafts to the Subject Coordinators for filing
b.11Print 3 copies of the checked Periodic Examination (1-for Risographing, 1-for Self-
Administration of the Test, and 1-for Subject Coordinators’ Files)
b.11 Secure the RISO Form from the Registrar’s Office and accomplish it
b.12 Submit the filled-out RISO Form together with the final copy of the test paper to the
Subject Coordinators for signature. The Subject Coordinators will submit the said
documents to the Assistant Principal. The AP will be the one to forward the signed
documents to the Duplicating Office.
ON Test Administration
The proctor shall:

1. report to his/her assigned class on or before the schedule time of examination.


2. check the attendance before the examination starts and list down the names of
students who are not able to take the examination for the day.
3. submit the list to the level supervisor for appropriate follow-up.
4. check the testing materials for any discrepancy of examination paper for each student.
5.distribute the examination paper by rows and instruct the students to fill-up the
personal details such as name, grade and section, date of examination and student
number.
6. read the general direction/instruction on how the test should be answered and remind
the students the time duration of the test.
7 .monitor the behavior of the students while taking the test and shall take appropriate
action on the misbehaviors observed like cheating or copying.
8. ensure that the students take the test properly and honestly.
9. remind the students five minutes before the end of test and shall ensure that everyone
is done answering on the schedule time.
10. require that the students submit the test papers by rows starting from the lastperson at
the back.
11. ensure that all test materials are accounted properly.
12. submit the test materials to the Subject Teachers or to the Registrar’s Office.

ON Checking of Examination Papers

1. Only the Subject Matter Experts (SME) shall check the examination paper.
2. Test scores shall be written inside the score box on the upper right hand corner of the
test paper.
3. Red ink shall be used in checking the test papers.
4. Correction on the test scores after the post review shall be initialed by the SME.
5. Encoding of final scores shall be done only after the post review.
6. The SME, upon receipt of the test paper, shall recount the number of test papers and
organize them for checking of items. The guide for checking of examination papers
shall be followed.

ON Recording of Test Scores

1. The Subject Matter Experts shall record the scores of the student in the approved
class record.
2. He/She shall encode the final scores after the post review in the Electronic Grading
System (EGS).

ON Test Result Analysis

1. The SME shall conduct the analysis of the test items covered in the periodic exam. He/
She shall prepare the profile of the class to include:
1.1 Number of students who answered correctly
1.2.Number of students who answered wrong
1..3 Items students found to be very difficult
1.2.4 Items students found to be very easy
2. The Subject Matter Expert shall get feedback from the students in the result of the test.
2.1 He/ She shall review the result of the items analysis and submit the copy to the
Subject Coordinators.
2.2 The Subject Coordinator shall review the test analysis result as to the:
2.2.1 Test construction
2.2.2 Time allotment
2.3 The SC shall conduct a meeting with the subject area teachers and discuss the
factors that may have affected the result of the test.
2.4 The SME shall make a plan of action to be considered for the next cycle of the
test construction.
2.5 The SME shall
2.5.1 reteach the subject matter which has the lowest scores.
2.5.2 re-construct the items which are easy if so the students answered
correctly.
2.5.3 discard the items which are very easy
2.6 The SC shall conduct a follow up in service workshop for test construction if
needed.

ON Releasing of Test Results

1. The Subject Matter Experts shall release the result of the test after the review and
rechecking.
2. He/ She shall ask the students to write the test score in their STUDENT
PERFORMANCE MONITORING SHEET.
3. Examination papers are not allowed to be brought home to retain their validity and
reliability.

C. Checked Test Papers


c.1 Alphabetically arrange the checked test papers.
c.2 On another sheet of paper, write the following as label:
c.2.1 School Year
c.2.2 Grading Period
c.2.3 name of SME
c.2.3 grade level, subject, and sections
c.3 Bind the test papers tightly with the label on top
c.4 Submit the bound test papers to the Subject Coordinators two weeks after the Periodic
Examination
