    This has reference to the Fire Safety Inspection conducted by _SFO2 GC Macaraeg
    (Name of Fire Safety Inspector)
    and FO3 LD Dumaguin with Inspection Order No. 2023- 369 on 21 March 2023.

    within your premises located at the above address wherein inspector’s report revealed the
    existence of the following defects/deficiencies, in the violation of the Fire Code of the Philippines
    of 2008 (R.A. 9514).

    No Fire Safety Maintenance Report on CLI w/in 15 days

    Submit Fire Safety Maintenance Report
    under Sec 9.0.3.4A Pp27 of Fire Code of the

    Failure to comply within the prescribe grace Php37,500.00
    period Notice to Correct Violation with Fine
    will be imposed.
    In this Regard you are hereby advised to comply /correct the above mentioned
    deficiencies within the above cited grace period otherwise appropriate Notice to Correct
    Violation with corresponding order to pay administrative fine shall be issued by this office.
