    do hereby depose and say:

    2. That the said inactive Gcash account has a remaining balance, amounting of
    Php10,000.00;

    3. That I am authorizing to transfer the amount of Php10,000.00 from my inactive GCASH


    account to my NEW GCASH account with the following details.
