    JOINT AFFIDAVIT OF DISINTERESTED PERSONS

    We, RYAN CHRISTIAN DELA TORRE and GERARDO RAMEL


    DELIMA JR., both of legal age, Filipinos and residents of LOT 23 BLK
    33 NORTHVILLE 4-A, LAMBAKIN, MARILAO, BULACAN, and LOT 30
    BLK 20 NORTHVILLE 4-A, LAMBAKIN, MARILAO, BULACAN, after
    having been duly sworn to in accordance with law do hereby depose
    and state THAT:

    1.     We have known MAR JUN MILAN BUTIAL for at least


    SEVETEEN (17) years, and know for a fact that his parents
    are __________________ and GLORIA  ROA MILAN,;
     
    2.     We, being friends  for more than SEVENTEEN (17) years
    we have known him and his parents specially his
    mother GLORIA ROA MILAN;
     
    3.     We know Mar Jun’s mother, for a fact as well, to
    be GLORIA ROA MILAN, since the early days, but
    discovered in the Birth Certificate of MAR JUN MILAN
    BUTIAL, that his mother’s name was registered
    as, GLORIA  ROA MILAN;
     
     
    4.     When a Certified True Copy of the Certificate of
    Birth of MAR JUN MILAN BUTIAL was requested from the
    then National Statistic Office now Philippine Statistics
    Authority, revealed that his mother’s name was
    erroneously registered to be  GLORIA  ROA MILAN;
     
     
    5.     We attest that MAR JUN MILAN BUTIAL’s mother’s
    first name is GLORIA, and that what was written in
    his Certificate of Birth was an erroneous entry. It was
    intimated that it was the midwife who assisted the mother
    of MAR JUN MILAN BUTIAL in giving birth who caused the
    registration of his birth and put entry on the details and
    names of the parents required in the application for
    registration of birth;

    6.     We attest to the truth of the foregoing facts


    that GLORIA ROA MILAN and NENITA  ROA MILAN, is
    one and the same person;
     
    7.     This Joint Affidavit is being executed to attest to the
    truth of the foregoing facts and to establish the identity
    of NENITA ROA MILAN  being the same person known as
    appearing in the Certificate of Birth of MAR JUN MILAN
    BUTIAL , as GLORIA  ROA MILAN;
     
    8.     This affidavit is executed for purposes of attesting the
    truth of the foregoing declaration that NENITA ROA
    MILAN  being the same person known as GLORIA  ROA
    MILAN in the Certificate of Birth of MAR JUN MILAN
    BUTIAL.
     
     
    IN WITNESS HEREOF we have set our hand this October 21, 2019, in
    Marilao, Bulacan.
