Kindly fill-out all required details in the registration link as it will be our basis for PRC submission. Please make sure to use your full name as registered to the Professional Regulatory Commission for the submission of attendance to PRC-CPDAS. Failure to do so may result to system rejection of PRC-CPDAS.
A confirmation email will be sent to your REGISTERED EMAIL after you completely submitted the Digital Registration.
Zoom Details, Evaluation Link, Reminders for the Convention will be sent to your email at least 1 week before the date of the actual Convention.
Please make sure you are using a WORKING EMAIL ADDRESS and double check your email before submission of the Registration Form. If you do not receive a confirmation email, kindly check your SPAM Messages. Please note that we will acknowledge ONE (1) EMAIL ADDRESS PER PARTICIPANT ONLY.

POLICIES ON REGISTRATION:
1. A clear copy of proof of payment should be attached to the digital registration form upon registration. It should be in a JPEG or pdf format with file size not bigger than 10 MB. 2. Use the Provided Link for Online Registration as it will be used as reference for the sending of zoom details, and the issuance of Official Receipt and Certificate of Participation. 3. Paid registration is transferable upon presentation of authority to transfer. 4. Official Receipt and Certificate of Attendance and Participation: ✓ Upon request, Official Receipt will be sent via payee’s registered EMAIL address WITHIN 10-15 working days. ✓ Original copies of the Official Receipt and Certificates will be sent to your respective chapter. If you wish to receive the documents on your individual address, you may inform us through email together with your mailing details. Shipping fee will be shouldered by the receiver. 5. Continuing Professional Development: Trainees and Participants are entitled to Certification of Participation with corresponding CPD points. To receive the Certificate of Participation, the Trainees/Participants should: i. Convention Attendance (Completed) ii. Convention Digital Evaluation (Completed) iii.
