MEMORANDUM OF AGREEMENT
Know all Men by these Presents:

This Memorandum of Agreement entered by and between ACES Polytechnic College, Inc., and
_____________ High School is intended to have strong and successful partnership under the Joint Delivery
Voucher Program (JDVP) with specific qualification on Organic Agriculture Production NCII (OAP NCII).
ACES POLYTECHNIC COLLEGE, INC. (APCI), is a private Higher Education Institution with business address
at San Francisco, Panabo City, Davao del Norte which is presently operating programs under the Technical
Education and Skills Development Authority (TESDA) and Senior High School program under the
Department of Education (DepEd), hereby represented by its President/CEO, DR. FRANCISCO P. DE LA
PEÑA, JR., and hereinafter referred to as the FIRST PARTY
- and -
___________________ NATIONAL HIGH SCHOOL, a government-owned and operated school with
business address at _______________ and presently offering Senior High School on Agri-Fishery
Programs hereby represented by its School Principal, ______________ and hereinafter referred to as the
SECOND PARTY.
WITNESSETH
WHEREAS, the FIRST PARTY has been serving the educational sector for the last 28 years and 5 years in
the JDVP in several qualifications with sterling performance;
WHEREAS, the FIRST PARTY has the Certificate of Program Registration from TESDA & DepEd of which
the SECOND PARTY intends to send its students for the JDVP under the Technical-Vocational Livelihood
(TVL) track;
WHEREAS, the SECOND PARTY is engaged in the education and training of the Senior High School (Grades
11 and 12) of which it needs the partnership of a private educational institution which can legally offer
qualification on Organic Agriculture NC II;
WHEREAS, the SECOND PARTY which has experienced the training delivery of the FIRST PARTY and has
the confidence that their students can develop skills and competencies that are both useful and gainful
after their training program; and
WHEREAS, both PARTIES have jointly believed that through their partnership the trainees will be able to
become productive citizens in their communities and in order to attain such, they will adhere voluntarily
to the following terms and conditions herein stipulated, to wit:
TERMS AND CONDITIONS

THE FIRST PARTY SHALL:
1. Prepare the training activities and deliver it to the students of the SECOND PARTY;
2. Conduct Training (mobile training & lock-in training);
3. Conduct Pre-assessment and assessment;
4. Provide insurance;
5. Supply FREE Round Trip Transportation, accommodation and meals (Breakfast, Lunch & Dinner)
during 6-7 day lock-in training with assessment;
6. Process the NC II certificate;
7. Provide certificate of training to the learners;
8. Submit completion report to the Department of Education through the school, division office and
regional office and for billing purposes; and
9. Do other functions as may be necessary to make this MOA successful.
THE SECOND PARTY SHALL:

1. Submit the names and codes of the qualified trainees to the FIRST PARTY as the basis of their official
acceptance;
2. Conduct periodic checking and monitoring of trainees as to their progress and development in
coordination with the FIRST PARTY;
3. Facilitate student participation during the conduct of FIRST PARTY’s review and assessment;
4. Enjoy FREE Round Trip Transportation during lock-in training with assessment;
5. Receive FREE Tool Kits;
6. Provide billing documents to the FIRST PARTY and ensure that there is no delay of such submission to
higher DEPED offices so that the FIRST PARTY is paid on time;
7. Keep all records and inform the FIRST PARTY if payments are made to tract delays and non-payment;
8. Assist the FIRST PARTY in following up payments and or status of billing to preclude the experiences
in the past;
9. Do other functions as may be necessary to make this MOA successful.
OTHER PROVISIONS:
1. That this MOA is in effect from the time of signing up to the time that the Department of Education
has fully paid the training and assessment fee commitment;
2. That the DEPED shall indicate the name of school and the amount paid to the FIRST PARTY to
avoid confusion and to track the unpaid balances; and
3. That the DEPED shall pay to the FIRST PARTY at least 2 months after the completion of training
and the billing documents are properly and fully submitted.
IN WITNESS HEREOF, We hereby affixed our signatures voluntarily this 27th day of April, 2022 at Panabo
City, Davao del Norte, Philippines.
