The undersigned group is undertaking a study entitled “Mastery Level of Accountancy,

Business and Management Tan of Tucdao National High School in Constructing Financial
Statements” as a part of the requirements in the subject Practical Research 2.
In connection to this, this self-appraisal form may contain words/phrase that you may find
ambiguous, you may ask the researcher or teacher to explain these words/phrases. The
researchers wants to give you a self-appraisal survey-test questionnaire relevant to the
researchers study that stated above.
The researchers will provide with a self-appraisal survey-test questionnaire that will focus on
your mastery level in creating financial statement. You will be asked of your personal

January 1, 2022 – Jumar started a merchandising business. He invested P5,000,000 cash in

the business.
January 2,2022 – He rented office space and paid for 4 months worth P4,000. (ASSET
METHOD)
February 3, 2022 – Purchase an equipment worth P150,000 and he sign a promissory note.
February 24,2022 – Purchase an office supplies P15,000.
March 25,2022 – Purchase merchandise from Rose Ann worth P350,000 with a term 3/10, n/30
FOB shipping point with an amount of P5,000.
April 1,2022 – Paid the whole amount to Rose Ann with a discount. It is the payment from March
25,2022 transaction.
April 10, 2022 – Sold merchandise to Louie worth P700,000 with a term 3/10, n/30 FOB
destination with an amount of P10,000. The delivery expense was already paid by Jumar with
cash.
April 12,2022 – Received from Louie with a discount. It is the receipt from April 10, 2022
transaction.
May 1,2022 – Jumar added new investment to the business worth P300,000.
May 13,2022 – Paid salaries to employees worth P20,000.
June 03,2022 – Paid utilities expense P10,000.
July 07,2022 – Purchase office supplies on account worth P20,000.
August 25,2022 – Jumar withdrew an amount of P50,000 from his business.
September 28,2022 – Paid taxes and licenses expense worth P10,000.
November 29,2022 – Consumed an office supplies worth P10,000.
December 31,2022 – Depreciation expense worth P5,000.
