    Team Responsibilities
    Incident Commander 1. Activate the Emergency Response team

    2. Activate additional response contractors and


    local resources

    3. Evaluate the Severity, Potential Impact, Safety


    Concerns, and Response Requirements based on
    the initial information provided by the First
    Person On-Scene

    4. Confirm safety aspects at site, including need


    for personal protective equipment, sources of
    ignition, and potential need for evacuation

    5. Communicate and provide incident briefings to


    company superiors, as appropriate

    6. Coordinate/complete additional internal and


    external notifications

    7. Communicate with Emergency Response


    Team, as the situation demands

    8. Direct response a cleanup operation

    Deputy Incident Commander 1. Ensuring that all emergency response team


    members are assigned duties and understand all
    emergency procedures.

    2. Working with other emergency response team


    members to evaluate an emergency

    3. Ensuring proper emergency communication

    4. Delegating needed emergency actions

    SHO/OSH-C 1. Liaises with Government Authorities (eg. Dept-


    of-Environment & Dept of Occ Safety & Health)

    2. Advise ERT on matters of HSE

    3. Update ‘Regulatory Board’

    Evacuation Warden (Subteam i) 1.Familiarization with their individual floor,


    including floor layout, location of emergency
    exits, stairwells and fire extinguishers.

    2. Maintain and update current rosters and lists


    for:
    o Emergency team members and alternates
    o All employees on their respective floors
    o All special needs individuals

    3. Provide current emergency team member roster


    to Director of Safety

    4. Remain in contact with building management


    to review emergency procedures

    1ST Aid (Subteam ii) 1.Keep team advised of any injury / serious injury
    or casualty

    2. Assists in treatment of casualties and render


    first aid as required

    3. Make arrangement for ambulance

    4. Report status to Emergency Commander

    Communication (Subteam iii) 1.Coordinate logistic request arrangement


    2. Coordinate material request arrangement

    3. Location (map) updating

    4. Advises on logistic issues

    5. Update ‘Logistic Event Board’

    6. Arrangement of requested transportation

    7. Co-ordinate material request

    8. Arrangement of domestic requirement -


    refreshment, etc.

    Logistic (Subteam iv) 1. Coordinate logistic request arrangement

    2. Coordinate material request arrangement

    3. Location (map) updating

    4. Advise on logistic issues

    5. Update ‘Logistic Event Board’

    6. Arrangement of requested transportation

    7. Co-ordinate material request

    8. Arrangement of domestic requirement -


    refreshment, etc.

    Fire Team (Subteam v) 1.Proceed to command post upon been notified


    the accident complete with fire safety equipment

    2. Perform fire evacuation procedure after been


    instructed by Incident Commander or Deputy
    Incident Commander.

    3. Once On-Scene report the situation to Incident


    Commander or Deputy Incident Commander.

    4. Try to put off fire if possible and immediately


    evacuate if the situation accelerated.

    5. Return to command post and handover the duty


    upon BOMBA arrival.
