Dear Mrs. Patel,

RE: Lauren Hill, 29 years old

I am writing to refer Ms. Lauren Hill for further psychiatric treatment. Ms. Hill was
admitted to the Psychiatric Ward in Royal Free Hospital on 29th September 2018.
Her admission was due to a drug overdose and suicide attempt. She will require
continued treatment at home.

Ms. Hill was found unconscious at home by her housemate and was suspected of
taking an overdose of diazepam. Upon arrival she was breathing heavily and not
responsive. She was treated with naloxone, which was ineffective. Blood tests were
run, and she received a gastric lavage. After 30 minutes she started to respond well.
She was given risperidone 4 mg, and this yielded positive results. Hourly neurological
observations were conducted as well as monitoring her vital signs. At the moment Ms.
Hill regrets her actions and affirms that she wants to live.

Taking into consideration the situation, I kindly request your assistance in providing Ms.
Hill with continued care and support. The medication was adjusted, she will start to
take risperidone 4mg at night time. In the first week she will have daily reviews to
guarantee medical compliance. As well as this, a referral to your local psychiatric team
for coping strategies at home will be needed.
