JOINT AFFIDAVIT OF LEGITIMATION

We, ROEL R. LAWAAN and ANACELI R. BARBO, both of legal age,


husband and wife and residing at 200 Kasunduan St., Brgy. Commonwealth,
Quezon City, after having been duly sworn to in accordance with the law, do
hereby depose and say:

1. That we have been living together as husband and wife without the
benefit of marriage before June 29, 1999;

2. That out of our relationship, we begot two children out of wedlock on


June 29, 1999 and June 19, 2000 at 200 Kasunduan St., Brgy. Commonwealth,
Quezon City and were named as BEVERLY BARBO and JOANA BARBO
respectively;

3. That the undersigned is the father of the above named-children and said
children were born during the minority of mother herein;

4. That when the mother herein attained the age of majority and there
being no legal impediment to marry, we contracted marriage on 18 August 2006
before the Regional Trial Court Judge of Quezon City. A xerox copy of our
Certificate of Marriage is hereto attached;

5. That by reason of subsequent marriage, we mutually acknowledge our


said legitimate children and we request the Local Civil Registry of Caloocan City
to make the proper annotation to carry the SURNAME of the father, in
accordance with Republic Act 9858 and other existing laws of the Philippines and
that their name be permanently recorded as Bevertly Barbo Lawaan and
JOANA Barbo Lawaan respectively.
