    JOINT AFFIDAVIT
    We, JESUS P. MANUEL, 63 years old, Filipino, married, and a resident of
    Bonifacio St., Poblacion G, Camiling, Tarlac, and ROSIE I. CHAVEZ, 55 years old,
    Filipina, married and a resident of Romulo St., Poblacion G, Camiling, Tarlac,
    Philippines, after having been duly sworn to in accordance with law, do hereby depose
    and state THAT:
    1. We both personally know MARVIN SOLON LAPITAN, born out of wedlock on
    December 28, 1997. Hence, the filiation of Marvin is Illegitimate. After the
    subsequent marriage of Marvins parents, Mario I. Lapitan and Emily B. Solon
    on January 26, 1998 and after the execution of the Affidavit of
    Acknowledgment of Admission of Paternity by my father, I became
    Legitimated. Machine copies of the Certificate of Live Birth of Marvin (with
    Annotation) and the Certificate of Marriage of Spouses Mario and Emily are
    hereto attached and marked as Annexes A and B;
    2. Lately, when Emily was scrutinizing the certificate of live birth of Marvin, she
    discovered that the recorded date of birth as appearing therein is MARCH
    14, 1937. A machine copy of Felicitas Certificate of Birth is hereto attached
    as Annex B. Hence, the discrepancy in the Date of Birth of Felicita as
    appearing in her SSS membership record;
    3. We are not related to Ms. Felicitas Santiago Aquino or to her parents by
    affinity or consanguinity except that of being their friends and acquaintances;
    4. We are executing this Affidavit to attest to the fact that the names FELICITAS
    AQUINO, who was born on March 14, 1937 appearing in the aforesaid
    Certificate of Birth, FELICITAS SANTIAGO AQUINO, whose date of birth is
    March 27, 1938, as recorded in her pertinent papers and transactions, and
    FELICITAS SANTIAGO AQUINO, who was allegedly born on March 27, 1937,
    as recorded in above-said SS E-4 Form and/or in the database of SSS, refer
    to one and the same persons, and that is our friend and acquaintance;
    5. Finally, I am executing this Affidavit to attest to the truth of the foregoing
    statements and for whatever legal purposes and intents this may serve.
    IN WITNESS WHEREOF, I have hereunto affixed my signature this 1 st day of
    October, 2013 at Camiling, Tarlac, Philippines.
