Terms and Conditions
 The Optical Network Unit (ONU) / Router, DigiBox and IP Set-Top-Box installed at the
subscriber’s unit for the implementation of FTDH Services remain the property of the
provider, unless purchased by the subscriber. Thus, it is imperative for the subscriber to
properly maintain the ONU and its accessories. The subscriber shall pay for the cost of
any units damaged due to his/her negligence. ONU device is covered by a limited one-
year warranty from the purchase date.
 Special implementation such as in-house roughing ins and any other configuration may
be separately billed to the subscriber.
 We require from the subscriber advance payment for Internet plan only, and both
advance payment and security deposit for bundled plans, Cable TV or IPTV plans.
 Billing shall commence at the date of acceptance as indicated in the Job Order Form. For
those existing subscribers who want to subscribe to the new plans, the effective date of
the new plan will start on the next billing period.
 All payments shall be made on or before due date through FTDH designated Bank
Accounts, electronic payments, through its Data Center or the company’s duly authorized
representative.
 Any unpaid bill beyond the payment terms shall be subject to monthly interest, penalty,
handling, admin fees and finance charges equal to 30% of the outstanding amount which
will commence from the due date.
 Payments shall be applied first to bills in arrears, including interest. The balance, if any,
will be applied to the current obligation.
 In addition to the fees and charges you incur in the normal use of service, we may charge
you for administration fee and other similar charges which may include late payment fees,
reconnection fees, downgrade fees, relocation fees, and service charge as may be stated
in your subscription plan. Other rates are applicable for commercial use such as Internet
Cafe, Realty Offices, and any other Small Office/Home Office Businesses inside Urban
Deca Homes.
 The company is authorized to offset/deduct any outstanding balance from the

subscriber’s deposit, should he/she fail to settle promptly his/her account.
 8990 FTDH may suspend the service without prior notice should they find:
a) That the subscriber fails to pay beyond the payment period;
b) If the subscriber is found out committing any unauthorized transfer or
tampering of the hardware/device/modem;
c) Allowing other person to transfer or tamper the hardware/device/modem
without prior consent from 8990 FTDH.
 FTDH is not an exclusive fixed line provider. All residents/tenants are free to apply to
other ISP’s. Should they apply to other networks, they are encouraged to settle their
outstanding obligation with FTDH to avoid unnecessary litigation.
 No lock-in period for the subscription and no penalties for disconnection will be in effect.
 It is agreed that the subscriber/unit owner shall be solidarily liable with the renter.
 8990 FTDH is not liable for any disruption due to fortuitous events caused by the
principal providers, or if the subscription applied is for residential plan but used for
commercial purposes, such as home-based or online business.
 No rebates are given in case of sudden power outage and or unexpected downtime from
main service providers.
 Prices are subject to change depending on the peso-dollar exchange rates and rates from
the primary providers.
 For CATV, Channel List or Add-on channels are subject to change without prior notice.
 These amended terms and conditions shall supersede the previously signed terms and
conditions for existing subscribers.
Subscriber’s Declaration
 I hereby confirm the foregoing information to be true and correct, and the supporting
documents genuine for the purpose of this application. I hereby authorize 8990 FTDH to
verify such information from whatever source including any amendment or modification
of the same to confirm its veracity in compliance with the Data Privacy Act of 2012, its
implementing rules and regulations, and any other issuances of the National Privacy
Commission of the Philippines.
 I hereby authorize the 8990 FTDH to use my information for communications related to
marketing or cross marketing only that will be needed to process my application and not
be given to outside entities or used for outside purposes, in compliance with the Data
Privacy Act of 2012.
 I hereby agree that violation of terms and conditions hereof shall already give 8990
FTDH the option to extra judicially cancel or rescind the contract which right 8990
FTDH reserves to exercise at the proper time.
 I am well aware of the following:

- Home Entertainment Burst to 5Mbps, with unlimited data
- Work from Home Burst to 20Mbps, with unlimited data
- Business Essential Up to 40Mbps, with unlimited data
(Average maximum speed range of 95% to 100% from 1:00am to 7:00am, 80% to
100% from 7:00am to 4:00pm, and 60% to 100% from 4:00pm to 1:00am.)
 8990 FTDH is authorized to inspect the installed FTDH devices inside the subscriber’s
unit for maintenance and support.
I hereby confirm that I have fully read and understood the terms and conditions of
this application and will strictly abide by them.
