1.0 OBJECTIVE

The objective of this plan is designed to help employees act properly in the
event of an emergency at the work area when they are working with VELOSI
SDN BHD and TERMINAL GAS TERENGGANU (TGAST), With this plan, the
employees is not panic, and they know what to do to save themselves and
property.

2.0 STATEMENT OF EMERGENCY

VELOSI shall give top priority to the safety and health of its employees and
to safeguard the environment. The utmost care shall be taken during
emergencies to protect or minimize damage to properties.

Staff shall be provided with appropriate personal protective equipment (PPE)


and training so that immediate and coordinated action can be taken to
manage an emergency.

Regular exercises and training shall be carried out to maintain the


awareness and effectiveness of the Emergency Response Procedures to
maintain a high level of preparedness should an emergency occur.

3.0 PURPOSE

To outline framework of the principles and method of on-site Emergency


Response Plan which ensure a practical, prompt, coordinated and
professional to an emergency during project, to minimize harmful effect on:
a. Human life and health
b. The environment
c. The site and surrounding assets
d. Company and corporate image

4.0 SCOPE
The basic procedures outlined in this document pertain to all employees
work at TGAST work site. This plan applies to all activities and personnel
related to TERMINAL GAS TERENGGANU It is oriented towards minimizing
initial impacts to the environment, health and safety of employees, the
community in the neighborhood and processes in the organization.

5.0 DEFINITIONS

OWNER : Shall mean PETRONAS CARIGALI SDN BHD is the owner of


PROJECT and include its employees, agents or designated
representatives.
TGAST : TERMINAL GAS TERENGGANU
VELOSI : VELOSI (M) Sdn Bhd
ERP : Emergency Response Plan
PTW : Permit to Work
HSE : Health, Safety & Environment

6.0 HAZARDS AND RISK ASSESSMENT


Hazards:

Main hazards for Inspection (ABSEILER TECHNIQUE) per below:


• Maintenance error e.g., wrong component – not fit for purpose
(Component failure)
• Working at height

7.0 RISK ASSESSMENT:

a) Rope access L3 shall conduct toolbox meeting prior to start work.


b) All workers need to identify and understand the working area before
starting the work.
c) Advise weather condition
d) All workers should carry suitable weight
e) All sharp edge must be identified and protected properly
f) All workers must be careful at the slippery floor area
g) All worker must always in 2 or 3 contact point position
h) All objects must be secure using lanyard

i) All battery operated / spark equipment must be inspected by TGAST


electrical team.
j) All authorized, defective equipment must get approval from the
TGAST team prior to start work.
k) Inspect PPE/Equipment daily prior to start work
l) IRATA L3 to assess and identify suitable anchorage point for rope
access.

8.0 MITIGATION PLAN

Based on the risk assessment, the following action need to be considered:


a) Comply with PCSB PTW procedure.
b) Ensure all workers are physically fit and medically certified by the
Occupational Health Doctor.
c) Authorized Work Leader shall brief all worker to understand hazard at
working area.
d) Use approved personnel gas detector.
e) Material, Equipment, and tool should be stored and arrange properly
f) Ensure all the equipment is inspected and in good condition.
g) Ensure proper housekeeping.
h) Use correct body posture.
i) All workers wear complete PPE as per IRATA procedure and
additional PPE instructed in PTW according to hazard.

1. All team members should fully understand the rescue plan if any incident
occurs.
2. In the event of a fall incident, regardless of the medical condition of the
employee, the standby person at bottom will direct an employee to call
emergency.
3. In the event an Abseiler is unable to descent himself to the ground or in
unconscious condition while rigging, the following step of rescue will be put into
action to ensure the victim receives adequate and fast response.
 IRATA Level 3 supervisor will approach the casualty as close as possible
 Rescuer will attach a separate rope and connect to casualty attachment
 Few types of rescues will be applied according to the situation.
 Casualty will straight be lowering to the ground
 Standby person/Supervisor prepare to evacuate casualty to the safe
place before emergency treatment given.
 All attachments will be taken out from casualty.
 First aid will be applied until the local emergency medical team arrives.
 The site and all equipment should be secure until a proper accident
investigation can be performed.

11.0 EMERGENCY EVACUATION

a. All persons involved in emergency responses shall comply with PCSB work
procedure and work instruction.
b. Personnel working within the plant shall be notified of an emergency through
activation of emergency alarm.
c. On hearing the emergency alarm, stop all work and shut off all sources of
ignition except for emergency siren test.
d. Vehicles shall be parked clear of traffic lane so that ERT can access properly to
the location of the emergency. Ignition key shall be left in the vehicle.
e. Look at windsocks to determine the wind direction and evacuate across the wind
or windward (upwind) of the affected area to the designated assembly point.
f. The head count shall be taken place at the identified assembly point.
g. All PTW shall be suspended during emergencies.

h. No one shall return to work unless “ALL CLEAR” alarm has been given and
PTW’s are revalidated.

11.1 Rescue Plan

When planning any work at height the Job Method Statement shall include a rescue
plan. Work at height shall not commence until the rescue plan is in place.

For working at height activities, the rescue plan shall be approved by the appropriate
approving authority.

The Rescue Plan should identify all potential incidents and emergency
procedures shall be established to deal with all such events. The rescue
plan should consider: -

Rescue shall also be assisted by IRATA LEVEL 3 and team.

11.1.1 Type of Rescue for the Job Task

Attachment – Rescue Photo Chart Should a technician go unconscious, then a


rescue operation shall be carried out. The Level 3 Supervisor or the nearest team
member will access the casualty, perform a snatch rescue, and descend down to
the ground together with the casualty.

12.0 RESPONSIBILITY

Authorized Supervisor (Level 3) Responsibilities:


• Shall brief all workers to understand hazard at working area.
• Select potential anchoring point
• Set up rope for the rope access activities.
• Always remain at the work site. -DO NOT LEAVE WORK SITE
• Maintain communication with all technicians.
• In an emergency, contact the work leader and stop work immediately.
• Rescue the casualty.
• When CEFS & TGAST (OSC) arrive inform them the number of casualties
and type of work being carried out etc.
• Assist CEFS as required. Comply to all IRATA guidelines.

Authorized work leader Responsibilities:


• Shall brief all workers to understand hazard at working area.
• Always remain at the work site. – DO NOT LEAVE WORK SITE.
• Maintain communication with the site supervisor (L3)

• In an emergency, contact TGAST person in charge and inform exact situation,


location, and severity of the emergency
• DO NOT attempt rescue.
• Help control movement of manpower in and out of the WAH area.

TGAST PERSON IN CHARGE


• Receive emergency notification from Authorized Work Leader.
• Notify CEFS FSP on site.
• Inform TGAST (osc) of the incident, location and situation involved, if any.
• Standby for furthers instruction from TGAST (osc).

TGAST On Scene Commander (OSC)


• Attend the work area.
• Assess the situation.
• Together with CEFS, TGAST (osc) formulate and implement overall strategy
for Rescue operation.

13.0 PROCEDURE
In an emergency, there are three procedures or methods that can be used. It
depends on the situation.

SITUATION 1

RESCUE CASUALTY WITH THE SAME ROPE (CASUALTY ROPE)

• This procedure will be used when the casualty was faint or totally can’t move
• Level 3 will transfer from their rope to the casualty rope.
• Level 3 will secure the casualty using their rescue abseiling gear.
• Casualty will be brought down safely by using the same rope.

• This procedure will be used when the casualty was very tired or panic during
the rope access activities.
• Level 3 will go to the casualty using their rope.
• Level 3 will go to the same level with the casualty.
• Level 3 will secure the casualty using their rescue abseiling gear.
• Casualty will be bringing down safely by using their rope.

14.0 Equipment Requirement and Standards

All rope access techniques and equipment used shall comply with the applicable
standards or guidelines, to ensure that all works are conducted in a safe manner.

The equipment shall be inspected by SSE (PCSB) prior to work commencing.
