    SWORN PROOF OF EXISTENCE

    We, the undersigned regional officers of the SALAAM PEOPLE’S


    ADVOCACY GROUP (SPAG) at Region XI, namely:

    (1) Taha N. Akmad, President, Region XI, of legal age and a


    resident of P-1, Brgy. Communal, Buhangin, Davao City;

    (2) Khursid A. Kaidar, Vice-President, Region XI, of legal age and


    a resident of Purok 75 Muslim Village, Brgy. Talomo, Davao City;

    (3) Rejal A. Hamisane, Secretary, Region XI, of legal age and a


    resident of Purok 75 Muslim Village, Brgy. Talomo, Davao City;

    (4) Danilo B. Garcia, Treasurer, Region XI, of legal age and a


    resident of P1, Blk. 35, Atis St., NHA Bangkal, Brgy. Talomo, Davao City;

    (5) Hafid M. Mama, Auditor, Region XI, of legal age and a resident
    of Purok 5 Poblacion Nabunturan, Davao de Oro;

    after having been duly sworn to in accordance with law, do hereby depose
    and state:

    1. We are the regional officers of SALAAM PEOPLE’S


    ADVOCACY GROUP (SPAG) REGION XI CHAPTER. A group picture
    of the regional officers is attached as ANNEX “A”.

    2. The regional office is located at beside Davao City Overland


    Transport Terminal, Davao City and the telephone/cellphone number is
    09265828162. A picture of said chapter office is attached as ANNEX “B”.

    3. The REGION XI CHAPTER of SALAAM PEOPLE’S


    ADVOCACY GROUP (SPAG) was formally organized on December 04,
    2021.

    4. To date, the REGION XI CHAPTER has more or less 2,000


    members and this number is growing as more membership applications are
    being processed daily.

    5. Since its formal organization, the SALAAM PEOPLE’S


    ADVOCACY GROUP (SPAG). has engaged in various activities for the
    benefit of the general membership and their families.
    6. We confirm that the SALAAM PEOPLE’S ADVOCACY GROUP
    (SPAG) first sought registration with the Securities and Exchange
    Commission (SEC) as a non-stock non-profit association on March 2, 2022
    using the corporate name SALAAM POLICE ADVOCACY GROUP and
    was granted the Certificate of Registration on May 25, 2022.

    7. We also confirm that we are made aware of the fact that SALAAM
    POLICE ADVOCACY GROUP (SPAG) is filing a petition for the
    accreditation of SALAAM PEOPLE’S ADVOCACY GROUP (SPAG) as a
    national sectoral party/organization under the party-list system and we are
    giving our consent thereto.

    8. We are executing this affidavit in order to attest to the truth of the


    foregoing circumstances, especially, to prove the existence of SALAAM
    PEOPLE’S ADVOCACY GROUP (SPAG) as a sectoral party/organization
    in REGION XI
