24-hr Dietary Recall Assignment Form-1
This is one type of tool for dietary assessment of an individual. For this method a Registered Dietitian or
a trained interviewer asks an individual to recall all foods consumed in the past 24 hours. The interviewer
should be non-judgmental while conducting the interview and try to prompt the patient for information on
brand names, portion sizes, recipe ingredients, cooking methods, condiments and beverages. Example of
questions: 1. What time did you get up yesterday? 2. What was the first thing you ate yesterday? How much?
3. When was the next time you ate? What did you eat? How much? 4. When did you eat next? What did you
eat? How much? 5. Did you eat or drink anything else between mealtimes? 6. Did you add any condiments –
butter, salad dressing, sugar? 7. Was this day’s intake different from usual? 8. Was this a weekday or a
weekend day?
A. DURATION:
• It is important to describe the following:
• The specific type of food you have eaten
• The time of day of the meal or snack was eaten
• The minutes you spent consuming the entire meal or snack.
D. CANNED FOODS: If you had a canned product, was it packed in water, its own juice, or
was syrup added to it?
E. CONDIMENTS: Ask if mayonnaise, mustard, catsup, steak sauce etc. was added to
appropriate foods. 2020
