INTRODUCTION
Community health nursing is a specialty area of nursing that focuses on the health and
well-being of people as individuals, families, and communities. In this area, nurses advocate
for community health needs, provide health education and counseling, prevent and manage
disease, manage health crisis, and coordinate care services. By taking into account social,
economic, cultural, and environmental issues, they operate in a variety of contexts and interact
with health care experts and neighborhood organizations to promote the general health of the
community. To improve the health of people, community health nursing places an emphasis on
holistic care and community-focused treatments.
We, the students nurses of the College of Maasin have scheduled a specific date to visit
Barangay Soro-Soro, Maasin City, Southern Leyte. During this visit, we will conduct household
surveys in the community. As student nurses and future health care providers, we will deliver
health care services to each household under the supervision of our Clinical Instructors.
This portfolio contains the data I collected during my Community Health Nursing
rotation. It includes information I gathered from interviews in each households. I have identified
a priority family, whom I will prioritize in delivering health promotion, teaching and management
services.

GENERAL OBJECTIVES
After three weeks of Community Health Nursing duty exposure in Barangay Soro-Soro,
Maasin City, Southern Leyte, I will be able to help the households of the barangay in terms of
understanding community needs, promoting health education, and collaborating with
interdisciplinary teams to improve overall community well-being.
SPECIFIC OBJECTIVES

INSIGHTS
Community Health Nursing is about understanding and addressing the unique health
care needs for communities. It emphasizes preventive care, health promotion, cultural
sensitivity, collaboration with diverse stakeholders, advocacy for vulnerable populations, data-
driven decision-making, effective communication, knowledge of public health policies, and
nurturing compassion.

BARANGAY HISTORY
The Barangay Soro-soro got it’s name from a plant called “SORO-SORO” that grows in
abundance in the area. The plant belongs to the cactus family. It is also used when cooking
beef, buffalo and horse meat. It is also used as medicine for injured skin diseases.
The Barangay Soro-soro has a total land area of two hundred and ten hectares (210
hectares). And the distance from Barangay Soro-soro to Poblacion is five meteres (5m).
The neighboring barangays: to the south will be Barangay Canturing, to the east
barangay hanginan, to the north barangay bagtican, and to the west barangay nasaug. The
total population of brangay soro soro is one thousand three hundred fifty eight. Based on the
RBI - Record of barangay inhabitants updates of 2020, the barangay consists of 318
households and consists of 340 families.

Community Health Nursing is a specialized field of nursing that focuses on the health
and well-being of communities, promoting health, preventing diseases, and providing health
care services within the context of the community.
The first day of our CHN rotation was all about orientation on what to do regarding CHN.
First, Sir CJ discussed about the requirements we need to comply for our CHN rotation and he
gave us the assessment tools to be used for the rotation. After that discussion, Sir CJ gave us
a break for us to take our snacks at the same time waiting for dean. When the dean arrrived,
she discussed the requirements and task for the rotation. After discussing, we took our lunch
at 12:30PM and instruct us to be back at 1:15PM. After taking lunch, we go back to the skills
lab and dean started to discussed about the Family Nursing Care Plan (FNCP) which was very
important for us because it was one of the contents of our requirements for CHN rotation.

Dean also gave us examples for this which understands me more and help me widen my
knowledge in doing FNCPs. After the discussion at 3:00PM, the duty shift ends.
It was all about orientation for our CHN rotation but the orientation totally give a big
impact especially for us as a first timers for this field. The discussion tackled by Sir CJ and
dean was abridge for us to enhance our knowledge more about CHN.

Community Health Nursing is a field that is totally tiring but at the same time very
productive. And it was based on my first ever experienced on this rotation.
It is our 2nd day of CHN Rotation. And in this day we are going to do our task in our
assigned Barangay which is Brgy. Soro-Soro Maasin City. We were informed by Sir CJ that the
call time was 8:00 A.M and our meet place will be at Soro-Soro Health Center. Upon arriving in
the Health Center, we were tasked first to do an ocular survey to the community with the
asisstance of Barangay Health Workers and also Sir CJ in order for us to know what were the
living conditions of their community. After the ocular survey, Sir CJ gave us the cue that we
can already start our interview for each household and each one of us was assigned to

interview 3 households. After that, me and my pair started to find households and also started
the interview. In the interview, we looked the number of people who lived in the home, their
education status, age, health problems as well as their living conditions, income, community
involvement and other relevant information.Through this informations, it was the basis we used
to figure out what was our health care focus. At exactly 11:05 A.M we finished interviewing the
first household and decided to go back to the Health Center as well as to took our lunch. After
lunch, Sir CJ checked our attendance first before we continue our interview for the next
household. After that we continue the interview and interviewed 3 households for that
afternoon. At 2:55 P.M we go back to the Health Center for the attendance and overall in that
day, we interviewed at least 4 households and me and my pair were only lacking 1 household
each which will be continued next week. After the attendance at 3:00 P.M. our duty shift ends.
Overall it was very fun at the same time tiring but a worthwhile experienced. The
important lesson I learned during this rotation that there were still communities facing poverty
that can lead to multiple challenges and one of this was the poor health outcomes associated
with poverty.

The whole duty for this day was very inspirable because of the last household I
interviewed the way they manage their life even though it was very rough. The learnings I
learned from this was that there was totally roller coasters in our life but at the end of the day
we still manage to reach the end point.

In this day, I’ve learned how to enhance my knowledge in making family nursing care
plan (FNCP) with the help four Clinical instructors and how important FNCP are.

Community health nursing was truly a tough and tired rotation as what I experienced
until our last day but the tiredness I felt was worth-having for.
During this day, we had our culmination day at the same time feeding program for the
daycare at Soro-Soro Barangay Gym. We arrived there very early so that we can all prepare
the things to do to make our feeding program successful. We are assigned by our Clinical
Instructors to group ourselves by committee in order to cater the needs as long as to make our
work more easier and convenient. And I was assigned in the Decoration Committee which was
to decorate for the feeding program. At 8:30AM some of the children accompanied with their
parents came early maybe because of excitement. At exactly 9:20AM we started our Feeding
Program. We also had health teaching about the proper hand washing and tooth brushing,
go,grow and glow foods and proper hygiene. This aspect was important especially to the
children as well as for the parents to inform what are the right ways to do to maintain the health

of their children. After the health teaching at exactly 11:00AM we distributed the foods for the
children. At 11:40AM the program ends. After the program our Clinical Instructors give
comments about the success of our program. At the same time discussing for the
requirements to be submitted. And at 11:50AM our duty ends.
It was very a tiring day but seeing the smiles of the children and parents push me to do
more and I am more inspired to continue my journey in nursing.

Community Health Nursing is a learned practice discipline with the ultimate goal of
contributing as individuals and collaboration with the others to the promotion of the client’s
optimum level of functioning through teaching and delivery of care.
During this day, we are going to have our gardening and health teaching to the priority
family. At exactly 10:00AM, we started our gardening in their mini garden just near the Soro-
Soro Barangay Health Center. I planted petsay seedlings and I am hoping that those seedling
would grow and bloom in order for them to harvest and use as their foods. Gardening plays an
important role in communities as their food alternatives when it times of financial crisis. In the
afternoon, we started our health teaching to each family we choose and give our knowledge to
the family to sustain their needs and have knowledge when their such of problems happen.
Health teaching is essential for prevention, early detection, and promoting healthy behaviors to
improve quality of life.

This day was our last day in community and I’ve learned a lot of things especially how
important health was and I will truly miss the experience being in community.
