ACKNOWLEDGMENTS
The creation of this diagnosis would not be possible without the aid of a few
notable people. We, the eleventh group of the first batch who completed their
Community Health Nursing duties, are grateful for Mrs. Miraluna and Ms. Isabel
Danielle Caturan’s tireless guidance and support, along with Mr. Christian Joshua
Caturan’s helping hand whenever it was needed the most. We would not have
To the other groups who shared the same success and failures with us,
encouraged us when we were not doing well enough or did not meet the expected
outcomes, helped us look for missing tools and data, and made the experience
more fun, thank you so much. You made the hard times less difficult to tread
against and we couldn’t be more grateful for the camaraderie that we had shared
with each other during the one-month duration that we were given to work together.
To the families that cooperated with us during our interviews, thank you for
sharing your thoughts, ideas, and experiences that helped us finish our duties
successfully.
And lastly, we are grateful to the Almighty God, for showering us with
wisdom, strength, and peace of mind in completing this task successfully. For His
guidance in helping us surpass all the challenges and trials, and also for giving us

Although different from the usual diagnosis encountered in the school and hospital,
community diagnosis shares the same nature as common diagnoses that aim to
deliver care to people who are less fortunate and are dependent on accessible
healthcare. This form of nursing is especially effective for people who live away
from the city where most healthcare services are easily available. It is also
significantly cost-effective and gives the clients freedom to continue their routines

Based on the data shown, it appears that the main issue in their community is
water supply because most of the areas are mountainous or near the sea, making
water supply inaccessible to all residents. The garbage comes next. The reason
for this is that the area around Barangay Guadalupe, Purok Lapu-Lapu is near the
