1. This award is given to recognize learners in Grade 12 who have exhibited exemplary skills and achievement in
science.
2. This award is given to learner/s who have high academic standing in science, demonstrated passion for Science
expressed through an excellent attitude toward Science work, and shown enthusiasm for Science, which possibly
impacts other students in class.
3. The academic rating for non-STEM classes that will be considered for this award is the student’s final grade in
CORE SUBJECTS - Earth and Life Science and Physical Science. For STEM classes, the CORE SUBJECTS are
Earth Science and Disaster Readiness and Risk Reduction (DRRR).
4. A candidate must satisfy ALL the required documents to qualify for this award.
5. A candidate must prepare a portfolio i.e. long-sized white folder fastened at the left side. The portfolio must include:
a. Letter of Intent of applying for the Award for Outstanding Performance in Science address to the Senior High
School Committee on Ranking and Awards Screening, Paul Christian R. Abad through its Award Chairperson
in Science, Oliver John M. Tres Reyes;
b. Latest Resumé with 2x2 picture (violet background);
c. Photocopies of Rubric and Evaluation Tool;
d. Summary of Final Ratings in the following subjects: Earth and Life Science and Physical Science (non-STEM
classes); Earth Science and Disaster Readiness and Risk Reduction (STEM classes). (Note: It should be
prepared and signed by the Class Adviser, if applicable);
e. Certificates as proof of membership or leadership (officer) to clubs or organizations related to Science;
f. Certificates with Pictures of Participation or Winning in discipline-related activities;
g. Commendation from Club or Organization Adviser and/or any Science Teacher in Junior and Senior High
School Departments;
h. Contribution to the School related to the discipline through accomplishment/narrative reports duly signed and
attested by the Adviser and Persons concerned.
6. All applications and portfolios must be submitted on or before 5:00 PM of April 30, 2024, to Oliver John M. Tres
Reyes, Award Chairperson in Science.
7. Any candidate/s who satisfy/ies 90% of the total rating shall automatically receive the Award for Outstanding
Performance in Science.
8. No application and/or additional support documents shall be accepted after the deadline.
