OUTSTANDING PERFORMANCE IN SCIENCE

GENERAL GUIDELINES FOR AWARD FOR OUTSTANDING PERFORMANCE IN SCIENCE

1. This award is given to recognize learners in Grade 12 who have exhibited exemplary skills and achievement in
science.
2. This award is given to learner/s who have high academic standing in science, demonstrated passion for Science
expressed through an excellent attitude toward Science work, and shown enthusiasm for Science, which possibly
impacts other students in class.
3. The academic rating for non-STEM classes that will be considered for this award is the student’s final grade in
CORE SUBJECTS - Earth and Life Science and Physical Science. For STEM classes, the CORE SUBJECTS are
Earth Science and Disaster Readiness and Risk Reduction (DRRR).
4. A candidate must satisfy ALL the required documents to qualify for this award.
5. A candidate must prepare a portfolio i.e. long-sized white folder fastened at the left side. The portfolio must include:
a. Letter of Intent of applying for the Award for Outstanding Performance in Science address to the Senior High
School Committee on Ranking and Awards Screening, Paul Christian R. Abad through its Award Chairperson
in Science, Oliver John M. Tres Reyes;
b. Latest Resumé with 2x2 picture (violet background);
c. Photocopies of Rubric and Evaluation Tool;
d. Summary of Final Ratings in the following subjects: Earth and Life Science and Physical Science (non-STEM
classes); Earth Science and Disaster Readiness and Risk Reduction (STEM classes). (Note: It should be
prepared and signed by the Class Adviser, if applicable);
e. Certificates as proof of membership or leadership (officer) to clubs or organizations related to Science;
f. Certificates with Pictures of Participation or Winning in discipline-related activities;
g. Commendation from Club or Organization Adviser and/or any Science Teacher in Junior and Senior High
School Departments;
h. Contribution to the School related to the discipline through accomplishment/narrative reports duly signed and
attested by the Adviser and Persons concerned.
6. All applications and portfolios must be submitted on or before 5:00 PM of April 30, 2024, to Oliver John M. Tres
Reyes, Award Chairperson in Science.
7. Any candidate/s who satisfy/ies 90% of the total rating shall automatically receive the Award for Outstanding
Performance in Science.
8. No application and/or additional support documents shall be accepted after the deadline.

OUTSTANDING PERFORMANCE IN SOCIAL SCIENCES

GUIDELINES FOR AWARD FOR OUTSTANDING PERFORMANCE IN SOCIAL SCIENCES


1. This award recognizes Grade 12 students in all strands in SHS who have exhibited exemplary skills and
achievement in Social Sciences.
2. This award is given to students with high academic standing in Social Sciences who consistently demonstrated the
willingness and ability to contribute and participate in activities that serve the common good by using their
knowledge, skills, and core values espoused in PERSONAL DEVELOPMENT and UNDERSTANDING CULTURE
SOCIETY AND POLITICS.
3. The students with an AVERAGE RATING OF 90% in PERSONAL DEVELOPMENT and UNDERSTANDING
CULTURE, SOCIETY, AND POLITICS are eligible to apply for this award.
4. A candidate must satisfy all the required documents to qualify for this award. He/she must prepare the following:
a. Resume with 1x1 picture (any background color);
b. Photocopy of School Form 9 (Learner Progress Report Card) back-to-back. Highlight the PERSONAL
DEVELOPMENT and UNDERSTANDING CULTURE, SOCIETY, AND POLITICS SUBJECT and the
corresponding GRADE;
c. Commendation from Club/Org Adviser and Personal Development and Understanding Culture, Society, and
Politics teachers;
d. Photocopy of Certificates as a Member or Officer of an Organization or Club;
e. Photocopy of Certificates of Participation or Winning in SOCIAL SCIENCES-RELATED ACTIVITIES; and
f. Proof of Contribution to the school community in line with the core values of the social science discipline as
reflected on the submitted Accomplishment/Narrative Reports duly attested by the adviser and PERSONS
CONCERNED.
5. All applications must be submitted on April 30, 2024, to the respective class adviser.
6. The class adviser will check the truthfulness and veracity of the submitted credentials and forward them to the
Committee Chairpersons on Social Sciences, Sol Cyber Ross A. Garcia and Sheena Aiza A. Bagacina.
7. Any candidate with 90% in the total rating of the evaluation committee shall automatically receive the AWARD FOR
OUSTANDING PERFORMANCE IN SOCIAL SCIENCE.
8. No application and/or additional support documents shall be accepted after the deadline. Incomplete portfolio will
not be processed.

2. This award is given to learner/s who have high academic standing in Mathematics, demonstrated passion for math
expressed through an excellent attitude toward math work, and shown enthusiasm for math, which possibly impacts
other students in class.
3. The academic rating that will be considered for this award is the student’s final grade in General Mathematics and
Statistics and Probability.
4. A candidate must satisfy ALL the required documents to qualify for this award.
5. A candidate must prepare a portfolio i.e. long size white folder fastened at the left side.
6. Portfolio must include a letter of intent indicating the intent to apply for an Award for Outstanding Performance in
Mathematics addressed to the Senior High School Committee on Ranking and Awards Selection through ALICIA

1. This award is given to bonafide learners in grade 12 who have exhibited exemplary skills and achievement in
Communication Arts.
2. This award is given to learner/s who have demonstrated proficiency in any language (Filipino, English, or other
foreign languages, written or oral communication, shown creativity in expressing ideas in written or oral activities in
various subjects, and contributed to the school community.
3. The academic rating that will be considered for this award is the student's final grade in Oral Communication,
Reading & Writing, English for Academic and Professional Purposes, Komunikasyon at Pananaliksik sa Wika at
Kulturang Pilipino, Pagbasa at Pagsusuri ng Iba't Ibang Teksto Tungo sa Pananaliksik, Filipino sa Piling Larang with
an average of AT LEAST 90 % with no grade BELOW 85% for the final grade.
4. A candidate must satisfy ALL the required documents and grade requirements to qualify for this award.
5. A candidate must prepare a portfolio, i.e., a LONG SIZE, WHITE FOLDER fastened at the LEFT SIDE. The portfolio
must include the following:

4. Qualified learners will be evaluated by the SELECTION COMMITTEE in their respective areas of specialization.

5. Candidate must prepare a portfolio long-size VIOLET plastic folder, fastened on the left side.
