establishing my identity and seeing it with UAN.

All the documents (Xerox) enclosed are singed by me and attested by employer.

Supporting documents required for category wise correction in PF records


Name correction:
• Latest e-Aadhaar card copy
• Anyone of SSLC(10th Class) marks card [or] Birth certificate [or] Indian Passport( Front & Back
page) copy.
• Name, Father’s name or Husband Name and DOB should be same in all the documents, if any
spellingdifference within the documents shall lead to rejection of application.
DOB correction:
• Latest e-Aadhaar card copy
• Anyone of SSLC(10th Class) marks card [or] Birth certificate [or] Indian Passport( Front & Back page) copy.
• PAN card copy.
• Name, Father’s name or Husband Name and DOB should be same in all the documents, if any
spellingdifference within the documents shall lead to rejection of application.
Father name correction:
• Latest e-Aadhaar card copy
• Anyone of SSLC(10th Class) marks card [or] Birth certificate [or] Indian Passport( Front- & Back-
page) copy.
• PAN card copy.
• Name, Father’s name or Husband Name and DOB should be same in all the documents, if any
spellingdifference within the documents shall lead to rejection of application.

Husband name correction:


• Latest e-Aadhaar card copy [after adding (W/o) husband name].
• Latest e-Aadhaar card copy of husband.
• Marriage certificate.
• PAN card copy with new name.
• Name, Father’s name or Husband Name and DOB should be same in all the documents, if any
spellingdifference within the documents shall lead to rejection of application.

Entire Name correction:


(Example: Mahesh Kumar (Old name) to Suresh Yadav (New Name) or Geetha (Old name) to Priya (New Name)
• Latest e-Aadhaar card copy (After changing to new name).
• PAN card copy with new name.
• Indian Passport(Front- & Back-page) copy with new Name.
• Name change gazette notification copy
• SSLC(10th Class) marks card or Birth certificate copy.
• Name, Father’s name or Husband Name and DOB should be same in all the documents, if any
spellingdifference within the documents shall lead to rejection of application.

Corrections in UAN created by Previous Employer


• Minor correction in Name / Father’s name / DOB less than 3 years (any one correction) can be initiated
• Major correction (Example : Mahesh Kumar (Old name/Fathers name ) to Suresh Yadav (New Name or
father’s name ) or Geetha (Old name) to Priya (New Name) or DOB from 06/03/1978 to 05/05/1990) and
multiple correction like name and Father’s name or name and DOB or father’s name and DOB is supposed
to be routed through previous Employer who created the UAN
