    Interview between me (as a midwife) and patient and make a medical report
    Patient : Good morning
    Midwife : Morning, please have your sit mr. and mrs. Can I might know who the mrs.
    name and where are you lived?
    Patient : Thank you. My name is Sandra and my husband is Toni. I live at Sentral
    street number 80.
    Midwife : Can I help you?
    Patient : I want to check my pregnancy
    Midwife : Ok Mrs. Sandra, Before that can I ask you a few questions?
    Patient : Sure
    Midwife : How old are you?
    Patient : I`m 29 years old
    Midwife : what is your religion?
    Patient : I`m a moslem
    Midwife : Do you have children before?
    Patient : Not yet, this is my first pregnancy
    Midwife : Alright, do you remember when the first day of your last menstruation
    periode is?
    Patient : yes, if I am not mistaken, it was November 8th 2023
    Midwife : when did you get the first time period? And how many days usually it
    happened?
    Patient : my first menarche is when I was 12 years old and my period usually 6- 7
    days.
    Midwife : Is there any problem during period like dysmenorrhoea or premenstrual
    syndrome ?
    Patient : no, I don’t have a problem
    Midwife : have you ever used contraception, if you have what type of contraceptive did
    you use before?
    Patient : I have used contraceptive pills for 6 months
    Midwife : are there any side effects ?
    Patient : no, I don’t have any side effect when I use pills
    Midwife : Okay, if I count from the first day of last menstruation, mrs Sandra already
    pregnancy 20 week. Do you have problem in this pregnancy?
    Patient : Well, I’m not sure if it’s morning sickness but I keep vomiting this whole
    week. I didn’t know I would have it in two trimesters.
    Midwife : Don’t worry, it’s actually normal to have morning sickness throughout your
    first and second trimester, any other symtoms you complain of?
    Patient : Heartburn, it hurts so much, especially when I have to throw up. It also made
    me lose a lot of weight
    Midwife : I need to know what you’ve been eating because this might have something
    to do with it. Let me guess, you did have problems with your stomach before
    you pregnancy, didn’t you?
    Patient : Yeah, I think I had a stomach disorder
    Midwife : Okay than, we’ll make sure you have a healthy diet after this
    Patient : So what should I eat?
    Midwife : You have to eat high protein and low fat foods, like for example is fruits and
    vegetables, it will help you
    Patient : Ok I will do your suggestion, thank you.
    Midwife : You’re welcome, are there any more questions?
    Patient : No I think it’s enough, thank you
    Midwife : No problem
