MUMBAI , MAHARASHTRA – 400010. Aadhar No. — 8388 5434
4886 do hereby solemnly affirm and sheweth following:

1. That I have been allotted Two (2) Permanent Account Numbers


(PAN) by the Income Tax Department and PANs ANJPS8870
and _____________.

2. That I want to retain my OLD Pan ANJPS8870Q for income tax


purposes and for all other purposes.

3. That I want to get Pan ______________ deleted with immediate


effect .

4. That PAN ______________ which is applied for deletion has


never been to my knowledge also used for any type of financial
transaction from the date of allotment of this PAN.

5. That none of my bank account is linked with this PAN.

6. That I have never utilized the PAN __________ for any lawful
purpose.

7. That this affidavit has been sheweth to the best of my knowledge


and belief.
8. That in case of any breach of above-mentioned declaration I am
liable for penalty under the income tax act or any other law
enforced in our country.
