INTRODUCTION

Fibromyalgia is a commonly encountered disorder characterized by chronic widespread


musculoskeletal pain,
pain, stiffness, paresthesia,
disturbed sleep, and easy fatigability along with multiple painful tender points, which are
widely and symmetrically distributed.
o Fibromyalgia affects predominantly women in a ratio of 9:1 compared to men.
o Newly
o Newly defined as a disease complex or syndromes

PREVALENCE

Wolfe 1995 reported the prevalence of FMS (all ages) to be 2% (females 3.4%, males
0.5%).Prevalence increased with age

It affects an estimated 3.7 million people in the United States (1), and women (most
often ranging in age from the mid-thirties to the late fifties) account for more than 75%
of patients

THE AMERICAN COLLEGE OF RHEUMATOLOGY CRITERIA:

A history of widespread pain for at least 3 months.


Pain is considered widespread when all of the following are present:
o pain in the left and right side of the body
o Below and above the waist.
o In addition there should be axial pain (cervical spine or anterior
chest or thoracic spine or low back).

Pain in 11 of 18 tender point sites on digital pressure.


o Occiput: bilateral, at the suboccipital muscle insertion
o Trapezius: bilateral, at the midpoint of the upper border
o Low cervical: bilateral, at the anterior aspect of the intertransverse
spaces at C5-7
o Supraspinatus: bilateral, at the origin, above the scapular spine
near the medial border.
o Second rib: bilateral, at the second costochondral junction, just
lateral to the junction, junction on the upper surface
o Lateral epicondyle: bilateral, 2 cm distal to the epicondyle
o Gluteal: bilateral, in the upper outer quadrant of the buttock
o Greater trochanter: bilateral, posterior to the trochanteric
prominence
o Knee: bilateral, at the medial fat pad proximal to the joint line.

PATHOPHYSIOLOGY

o Changes in neuroendocrine transmitters such as serotonin,


substance P, growth hormone and cortisol suggest that regulation
of the autonomic and neuro-endocrine system appears to be the
basis of the syndrome.
o Central sensitization, blunting of inhibitory pain pathways and
alterations in neurotransmitters lead to aberrant neuro-chemical
processing of sensory signals in the CNS, thus lowering the
threshold of pain and amplification of normal sensory signals
causing constant pain.
o The frequent co-morbidity of fibromyalgia with mood disorders
suggests a major role for the stress response and for
neuroendocrine abnormalities.
o stress adaptation response is disturbed.
o Can be triggered by physical, emotional or environmental stressors
such as car accidents, repetitive injuries and certain diseases
o Sleep deprivation with disruption of delta-wave sleep (non-REM
stage IV) is associated with daytime fatigue and fibromyalgia
syndrome.
o FMS could be a neuroendocrine disturbance, particularly involving
thyroid hormone imbalances and/or hypophyseal growth hormone
imbalances.

DIAGNOSIS

o A diagnosis is made by evaluation of symptoms and presence of


tender points
o American College of Rheumatology Classification Criteria for
Fibromyalgia (1990)...... widespread pain for at least 3 months
and pain in 11 out of 18 tender point sites on digital palpation.
o Both criteria must be satisfied
History of widespread pain for more than 3 months, on both sides
of the body, above and below the waist, and axial skeleton
(cervical spine, anterior chest, thoracic pain, or low back)
Pain in 11 of 18 tender point sites on digital palpation with
approximate force of 4 kg.
o X-rays, blood tests, specialised scans such as nuclear medicine and
CT, muscle biopsies are all
normal
o ESR (erythrocyte sedimentation rate) is normal

RHUS TOXICODENDRON

INTRODUCTION

• Rhus Tox is a top listed medicine for treating fibromyalgia. It is


indicated in cases where there is widespread pain in the body.
The pain is present in the muscles, tendons, and ligaments and
can be tearing, shooting, stitching in nature.

• A peculiarity of the plant is that it is more poisonous during the


night, and when bursting into leaf, or at any time in June or July
when the sun is not shining upon it. Absence of sunlight, together
with dampness, seems to favor the exhalation of Toxicodendric
acid.

• the severest symptoms and sufferings are excited when the body
or the limb is at rest, and kept as much as possible without
movement.

• It is seen to get worse with exertion and during the night. The
sleep gets disturbed, and there is marked stiffness in the body.
Back pain is very prominent, and it gets better with motion.

Key indications for using Rhus Tox for Fibromyalgia:

o Widespread pain in the body.

PARTICULAR SYMPTOMS

• Rheumatic stiffness of nape and neck, with painful tension during


movement.─Painful swelling of axillary glands.─Rheumatic
tearing between scapulæ, not affected by movement, < by cold, >
by heat.─Tearing between shoulders, drawing together from both
sides.─Transient coldness in back.─Contusive pain in loins, esp.
on touching the parts, and during repose.─Pain in small of back
when sitting still or when lying; > when lying on something hard or
from exercise.─Bruised pain in sacrum when he lies still on it or
sits still; feels nothing of it when moving.─Painful rigidity in
loins.─Painful exostosis on sacrum.─Distortion of the vertebral

• UPPER LIMB :

• LOWER LIMB :

Eruption with sweat in cleft of nates.).─Aching pains in legs; must


change position every moment.─(Pains in l. lower limb, thigh
chiefly, from septic absorption in old abdominal disease, with
vesical irritation.─R. T. C.).─Shootings and tearings in hip-joint,
extending to ham, esp. when resting on foot; or with dull
drawings and burning sensation during repose, and painful
sensibility of joints on rising from a seat or on going up stairs (or
other over-exercise; involuntary limping).─Tension and stiffness
of the muscles and joints of hips, thighs, legs, knees, and
feet.─Paralysis of the lower extremities.─Cramp in calf after
midnight, when lying in bed, and when seated after walking; it

MODALITIES

Aggravation: Stiffness and pains are worse on initial motion. Complains


are brought about by over-lifting, over-work or over-exertion.

Amelioration: better by continuous motion

ARANEA DIADEMA

INTRODUCTION

• Aranea diadena Is the Indicated remedy for fibromyalgia- Indicated for


Hydrogenoid Constitution, i.e, Abnormal sensitiveness to damp and
cold, inability to live near fresh water, lakes, rivers, etc, or in damp,
chilly places

• All symptoms of Aranea are characterized by periodicity, and coldness,


and great susceptibility to dampness.

• Patient feels cold to the very bones. Coldness not relieved by anything.
Feeling as if parts were enlarged and heavier. Wake up at night with
hands feeling twice their natural size.

• Bone-pains in extremities. Pain in os calcis. Sensation of swelling, and


of parts going to sleep.

UPPER LIMB

• Numbness of parts supplied by ulnar nerve.─Dull, penetrating pains
in the bones and in all parts of the body, esp. in the humerus and the
forearm.─At night a sensation as if the hands and forearms were
larger and heavier.─Sensation on the ring and little fingers as if they
were asleep.

LOWER LIMB
• Boring and digging in the r. tibia and os calcis, continuing on moving
the foot, and disappearing by continuing the motion.─Ulcer on the l.
heel.

MODALITIES

AGGRAVATION .--damp weather; late in afternoon, and at midnight.


AMELIORATION -- smoking tobacco.

OTHER INDICATED HOMEOPATHIC REMEDIES FOR FIBROMYALGIA

Arnica montana

• Arnica Montana is a natural medicine for fibromyalgia prepared from


the Leopard’s Bane plant. It belongs to the family Composite.
• It is primarily used in cases of fibromyalgia where there are sore,
bruised pains in the body.
• The body feels lame and sore as if beaten.
• It is sensitive to touch, and the person feels afraid to touch the
painful parts. Fatigue, numbness, and tingling of limbs may be
present.

KEY INDICATION :
- Sore, bruised pains.
- Fatigue with body aches.
- Numbness and tingling in limbs.

KALI PHOS

• Kali Phos is a natural remedy for fibromyalgia used to treat marked


fatigue and sleep issues in cases of fibromyalgia.
• There is an intense weakness, tiredness and whole system feels
drained out.
• The affected person gets easily fatigued and has body aches.
Exertion worsens the condition.
• Sleeplessness is another major issue that is treated well with this
medicine.
• A prickling sensation in the hands and feet and fibromyalgia linked
with stress are also indicative of this remedy

BRYONIA ALBA

• Bryonia Alba is a natural medicine prepared from the plant Wild

Hops. The natural order of this plant is Cucurbitaceae.
• Bryonia Alba is a very beneficial medicine for fibromyalgia
considered in cases where pains get worse from motion. Lying down
and absolute rest offers relief in pains.
• The type of pain varies from sharp, stitching, bursting, sore to
tearing in nature.
• Pressure causes pain on tender points founds on the lower back and
knee. Stiffness also attends the pain.

MAG PHOS

• Magnesia Phos is a well-indicated natural treatment for fibromyalgia


where warmth relieves the pains.
• The pains in the body are mostly shifting in nature, changing from
one place to other frequently.
• The character of the pain is mostly sharp and shooting. Light touch
tends to worsen the pain, and there is a pain in the neck, lower
back, and lower limbs.
• The pains also tend to radiate from the point of origin. Restlessness
and extreme weakness are present, and cold tends to worsen the
pains.

Ruta Graveolens

• Ruta Graveolens is a remedy for fibromyalgia prepared from the
fresh plant of Garden Rue of the family Rutaceae.
• It is indicated for pains in the body along with restlessness. Soreness
of tendons and pains are of deep, sharp, drawing, tearing in nature
are mainly present in the neck, shoulders, lower back, and knee.

ARSENICUM ALBUM

• Arsenic Album is a useful medicine for fibromyalgia with marked


fatigue.
• The key indications for using this medicine include sudden weakness,
excessive tiredness, exhaustion from slight exertion, burning pains
in the body in areas like the back and limbs.
• Use of this medicine is also considered in cases of fibromyalgia when
intense anxiety (particularly about health) and restlessness is
present.
• All the complaints are seen to get worse during the night.

CIMICIFUGA RACEMOSA

• Cimicifuga Racemosa is indicated for fibromyalgia with marked pains


in the muscles of the neck and the lower back.
• A feeling of stiffness and contraction in the neck is also present.
• Dull, heavy aching in the lower back that is improved by rest, pains
in muscles worsening from motion and excessive muscular soreness
are other accompanying symptoms.

GELSEMIUM SEMPERVIRENS

• Gelsemium Sempervirens is prepared from the plant Yellow Jasmine.


• The natural order of this plant is Loganiaceae.
• Key indications for the use of this medicine include aching in
muscles and tiredness, deep-seated pains that are worse in the back,
hips, neck, and under the scapula.
• Exertions worsen the pain and headaches may also be present along
with the above symptoms.

COFFEA CRUDA

• Coffea Cruda is a medicine used to treat sleep issues in fibromyalgia


• The person needing this medicine has severe insomnia (sleeplessness)
• It is indicated in cases of restlessness and disturbed sleep.
Hypersensitivity towards pain is another common feature.

MATERIAL AND METHODOLOGY

1. STUDY SETTING:
Study was conducted on the cases selected from the O.P.D. of Sainath
homoeopathic hospital attached with the Rajkot homoeopathic college
affiliated with parul university.
2. DURATION: 6 months.
3. SAMPLE SIZE: 25
4. SELECTION OF SAMPLE: Randomly selected cases.
5. INCLUSION CRITERIA:
Cases were included irrespective of age, sex, caste and religion
6. EXCLUSION CRITERIA:
Patient who had not given written consent.
In the cases with severe pathological conditions.
7. INFORMED CONSENT:
Written consent was taken from the patient for the study.
8. STUDY DESIGN:
A prospective experimental clinical trial.
9. SELECTION OF TOOLS:
Standard case taking Proforma along with consent form was used.
10.DATA COLLECTION:
All data were kept in the form of hard and soft copy.
11.ETHICAL ISSUES:
Ethical clearance was taken from the ethical committee of the institute
prior to conducting the study

From the above work and discussion-it has been observed and notified that
RHUS TOX AND ARANEA DIADEMA works its Best for not only Fibromyalgia but
also for other types of the myalgia

SUMMARY AND CONCLUSION

SUMMARY

Based on that cases we find some effects of Rhus Toxicodendron and Arena
Diadena in cases of Fibromyalgia .

Based on the case series there are 29 % case with marked improvement, 36%
moderate , 21%mild and14% patient having no improvement after use of Rhus
Tox or Aranea Diadema in Fibromyelgia .

CONCLUSION

So, whenever it is not possible to find out exact similimum or any


constitutional remedies or absence of symptoms for fibromyalgia at that time
Rhus Tox and aranea diadema has been helpful.We need to do further
studies on this to find the Utility of Rhus tox and aranea diadema in cases of
fibromyalgia
