CHAPTER I

INTRODUCTION

Introduced in 2009, CHED Memorandum Order 23 serves as a framework for the


implementation of OJT programs in higher education institutions (HEIs) throughout the
Philippines. This policy aims to enhance the employability of graduates by fostering a seamless
transition from academia to the professional world. By integrating practical training into the
curriculum, students gain valuable workplace skills, industry knowledge, and relevant
experiences that complement their theoretical learning.
The inclusion of On-the-Job Training (OJT) in the educational system of the Philippines
has emerged as a response to the growing recognition of the need to bridge the gap between
academia and industry. In a rapidly evolving job market, it has become apparent that graduates
must possess not only theoretical knowledge but also practical skills and competencies to
succeed. Consequently, the OJT program has gained significant popularity as an effective
method to equip students with the necessary tools for their future careers.
Among the many educational institutions in the Philippines that adhere to CHED
Memorandum Order 23 is the Cagayan State University (CSU) Sanchez Mira Campus. Located
in the province of Cagayan, CSU Sanchez Mira Campus recognizes the importance of OJT in
preparing students for future careers. The implementation of OJT at CSU Sanchez Mira
Campus reflects the institution's commitment to producing well-rounded graduates who are
equipped with both academic knowledge and practical expertise.
Cagayan State University Sanchez Mira Campus, renowned for its dedication to
academic excellence, has wholeheartedly embraced the OJT program as a means to enhance

OBJECTIVES

Information technology is building communications networks for a company, safeguarding


data and information, creating and administering databases, helping employees troubleshoot
problems with their computers or mobile devices, or doing a range of other work to ensure the
efficiency and security of business information systems.
In order to provide the Bachelor of Science in Information Technology students in the
Cagayan State University – Sanchez Mira Campus with opportunity to complement their
formal learning with practical knowledge, skills and desirable attitudes and to gain hands on
experience in the industry, a 500-hour internship program was developed and integrated in the
program’s curriculum.
As per the CMO No. 104, Series of 2017, the objectives of the Internship Program for all
Programs in the Philippines are outlined as outcomes which present the competencies that
HEIs, Students, and HTEs must exhibit at the end of the internship program. These are the
following:

General Objectives:
1. Strengthen the implementation of the local internship through collaborative effort of the
academe-industry linkage among the participating HEIs and partner
institution/establishment;
2. Establish the harmonious collaboration/linkages between higher education institutions
(HEIs) and the Host Training Establishments (HTES) for the effective implementation of
the program to support the educational environment of the student interns; and
3. Train student interns under actual workplace conditions.

Specific Objectives for:


Higher Education Institution (HEI)
a. Enrich the degree programs in HEIs to respond to the needs of the industry;
b. Promote mutually supportive industry academe collaboration/linkages; and
c. Strengthen career guidance.

Student Intern
a. Provide students enrolled in HEIs in the Philippines with opportunities to apply
relevant knowledge and skills acquired from formal education to actual work setting
provided by reputable HTEs in the country.
b. Enhance the knowledge and skill acquired in formal education of student interns
through employer employer-based training, in order for them to become more
responsive to the future demands of the labor market;
c. Develop the life skills of the student interns, including those relevant to the values of
professionalism and work appreciation. The student interns are expected to acquire soft
skills necessary to address the demand of the employers such as communication skills,
interpersonal skills, financial literacy, etc.; and
d. Acquire a professional work ethic in the course of internship.

Host Training Establishment (HTE)


a. Provide employers avenue where they can customize the process of technical training
through employer-driven internship plans; select well-equipped beneficiaries and have
opportunity to contribute to the society through corporate social responsibilities.

The SSS Laoag City branch, part of the Social Security System in the Philippines,

security and welfare. The esteemed Branch Head, Richard Raralio, leads the team, overseeing
the smooth operations and efficient delivery of services.
To reach the SSS Laoag City branch, individuals can contact the office through the

serve as a direct line of communication for inquiries, assistance, and any necessary
transactions related to SSS services.

The branch caters to a range of services, including loan applications, online inquiries,
online registration, and SSS contribution matters. Members can approach the office for
assistance with their loan applications, seek information and updates regarding their SSS
accounts through online inquiry services, register for online access to manage their accounts,
and address queries or concerns related to their contributions.

Narrative Report of Weeek 1


It is Monday, February 13, 2023, and this marked the beginning of our venture into the
illustrious SSS building. As we stepped foot into the premises, a surge of emotions flooded my
being, encompassing both excitement and trepidation. The anticipation of what lay ahead
propelled me forward, eager to embark on this new chapter.
To our surprise and delight, we were graciously greeted by none other than the esteemed
branch head himself, Sir Richard Raralio. His warm welcome instantly put us at ease, setting a
positive tone for our inaugural experience within the SSS building.
The initial order of business involved a comprehensive orientation conducted by Ma'am
Roma Amor Aguinaldo, the junior administrative assistant. With utmost professionalism and
proficiency, she meticulously outlined the various rules and regulations that governed the
operations within the office. Making the experience even more gratifying, Ma'am Roma was
accompanied by Ma'am Dana Agtang, the accomplished CEO II of the SSS Laoag Branch,
showcasing the institution's commitment to ensuring a smooth transition for newcomers.
After the enlightening orientation, we were fortunate enough to have Ma'am Roma as
our dedicated guide, accompanying us on an extensive tour of the offices. Her amiable
demeanor and deep knowledge of the organization allowed her to introduce us to each staff
member, shedding light on their roles and responsibilities. It was an invaluable opportunity to
familiarize ourselves with the SSS community, fostering a sense of connection and camaraderie
from the very beginning.
Within this vast and bustling institution, the Accounts Management Section emerged
as our designated haven. Among the numerous individuals present, Tricia, Hanna, Nikki,
Anthony, and I were specifically chosen to be integrated into their team for the duration of our
stay. This allocation instilled a sense of belonging and purpose, as we became part of a closely-
knit group dedicated to managing accounts and ensuring a seamless experience for SSS
beneficiaries.
As fate would have it, I found myself under the guidance of Ma'am Jan Marie P.
Casabay, a junior analyst known for her astute insights and analytical prowess. With her wealth
of knowledge and expertise, I knew that being assigned to her was a tremendous opportunity
for personal growth and development. Excitement mingled with a touch of nervousness as I
prepared to learn from her, eager to contribute to the SSS's mission under her tutelage.
Thus, on this momentous Monday, February 13, 2023, my journey through the SSS
building commenced. With a blend of emotions coursing through my veins, I embraced the
challenges and possibilities that awaited me, fueled by the warm reception, comprehensive
orientation, and the camaraderie of newfound colleagues.
During our first week at the SSS Laoag Branch, we are filled with a sense of awe and
determination as we engage in our on-the-job training. The experience of actively participating
in tasks and responsibilities within this esteemed organization feels surreal but inspiring. As
new trainees, we encounter unfamiliar challenges and tasks, and the learning curve can be
overwhelming. However, our supervisors demonstrate exceptional patience and understanding,
providing us with thorough guidance and support. They break down complex procedures,
explain concepts, and ensure we grasp the reasoning behind each task, boosting our confidence.

Solutions:
I brought my personal laptop to the office to address the shortage of laptops and provide
an additional device when all of us needed to work in front of a monitor. This proactive action
ensured a seamless workflow, facilitating collaborative tasks and granting equal opportunities

During the second week of my tenure at the SSS Laoag City branch, I was fortunate to
encounter several noteworthy experiences that left a lasting impression. One particular
highlight was when Ma'am Michelle entrusted me with the important task of designing a
captivating flyer that effectively showcased the diverse range of services offered by the SSS.
This assignment not only provided me with an opportunity to unleash my creativity and
demonstrate my graphic design skills, but it also allowed me to play a role in promoting the
valuable services provided by the SSS. It was truly fulfilling to contribute to the dissemination
of crucial information and engage with potential SSS members, knowing that my efforts could
potentially benefit numerous individuals.
Throughout the week, my primary focus centered around payroll processing and
handling incoming telephone calls from clients. With meticulous attention to detail, I dedicated
myself to the accurate and efficient execution of payroll tasks, ensuring that all calculations
and procedures adhered to the established guidelines. Moreover, I approached each client
interaction with a high level of professionalism, actively listening to their inquiries and
concerns, and providing them with the necessary assistance and guidance. It was gratifying to
play a role in resolving clients' issues and leaving a positive impression that fostered trust and
satisfaction.
In addition to my core responsibilities, I took the initiative to familiarize myself with
the different sections and departments within the office. By proactively seeking understanding
about the specific purposes and functions of each department, I gained a comprehensive
overview of the office's intricate operations. This knowledge not only facilitated more efficient

During my time in the Accounts Management Section (AMS), I encountered numerous
tasks that were completely new to me which is a big problem. One such task that stood out was
payroll audit. When Ma'am Jan assigned me the responsibility of auditing the payroll of a
specific company, I initially faced a challenge as I lacked the necessary knowledge and
experience to perform the task proficiently. However, Ma'am Jan came to my aid, taking the
time to patiently teach me the intricacies of payroll auditing, particularly focusing on the
formulas involved. Her meticulous guidance and thorough instruction ensured that I gained a
solid understanding of the process, allowing me to carry out the payroll audit effectively.
Through Ma'am Jan's mentorship, I not only acquired a valuable skill in payroll auditing but
also developed a deeper appreciation for the importance of patient and knowledgeable guidance
in the workplace.
I also proactively endeavored to acquaint myself with the various sections and
departments present in the office. By actively seeking knowledge about the unique objectives
and functions of each department, I obtained a comprehensive understanding of the intricate

During the third week of my internship at SSS Laoag City, my focus was primarily
directed towards the challenging task of redesigning the flyer. This undertaking required a
significant investment of time and effort as I strived to refine the design, ensuring that it would
effectively convey the desired message and captivate the target audience. Recognizing the
value of expert guidance, I actively sought feedback and sought out regular meetings with
Ma'am Michelle, who provided invaluable insights and critiques to enhance the flyer's quality.
Throughout the week, I engaged in a continuous cycle of presenting my design
revisions to Ma'am Michelle, eagerly awaiting her constructive criticisms, clarifications, and
suggestions. Her expertise and knowledge served as a guiding light, illuminating the path
towards an improved and impactful flyer. I valued her input as it played a pivotal role in
refining the design and aligning it with the organization's goals and expectations.
Among the primary challenges I encountered during the redesign process was striking
the delicate balance between integrating additional content into the design while preserving the
overall concept and visual harmony. This task demanded a careful and meticulous approach,
requiring me to carefully evaluate the placement and integration of new information. I learned
to appreciate the importance of maintaining a cohesive and unified design while effectively
incorporating relevant details to enhance the flyer's value and appeal.
Upon completing the final design of my initial draft, I eagerly proceeded to print it out
and submitted it to Ma'am Michelle for finalization. However, during the review process, I was
disheartened to discover an error that had gone unnoticed. This humbling experience served as
a powerful reminder of the importance of thoroughness and attention to detail in the design
process. Instead of succumbing to frustration, I embraced the mistake as an opportunity for
growth and improvement.
With determination and a commitment to excellence, I promptly set out to rectify the
error and made the necessary adjustments to align the design with the desired specifications
and objectives. I meticulously reviewed every element, ensuring that each detail contributed to
the overall effectiveness of the flyer. This process instilled in me a deep appreciation for the
significance of resilience, adaptability, and continuous learning in the face of setbacks.

requirement as some of the existing staff members possess the skills to manipulate graphics.
While there isn't a specific role solely focused on graphic design, the office recognizes the
importance of visual elements in their work and encourages staff members to utilize their
graphic design skills as needed. This approach allows for flexibility and collaboration within
the team, where individuals with graphic design knowledge can contribute in enhancing the
visual aspects of various projects and materials. By leveraging the existing skills of the staff,
the office is able to meet their graphic design needs without the necessity of hiring a dedicated
graphic designer.

Solutions:
Fortunately, during my OJT experience in their office, I had the opportunity to
showcase my potential in graphic design, and I am grateful that they placed their trust in me.
This mutual trust between the office and myself allowed us to harness the talent and skills
available internally, eliminating the need to seek designs from external sources. By recognizing
and utilizing the capabilities within the office, we are able to produce high-quality designs that
meet their requirements without incurring additional costs or relying on external vendors. This
not only demonstrates the office's resourcefulness and efficiency but also highlights the value
of nurturing and leveraging internal talent to achieve desired outcomes.

After dedicating several weeks to the meticulous process of designing a captivating and
informative flyer, I once again found myself immersed in a new and exciting design project.
This time, I was entrusted with the responsibility of creating a visually appealing banner for
the esteemed KaltaSSS Collect Program, an initiative offered by the SSS that extends
comprehensive social security protection to self-employed individuals and qualified Filipino
workers. This invaluable program serves as a safety net, guarding individuals against a myriad
of potential contingencies, including disability, sickness, maternity, old age, and even the
inevitability of death.
In addition to my design responsibilities, Ma'am Jan entrusted me with a task of
significant importance—manually documenting and organizing the inventory of file records
for endorsement. This endeavor necessitated meticulous attention to detail as I diligently sorted
through an extensive collection of employer records sourced from various municipalities,
including Laoag, Paoay, Batac, Pinili, Badoc, and Currimao. The sheer volume of 120
employer records presented an initially daunting prospect. Nevertheless, I approached the task
with determination and resolve, recognizing the paramount significance of maintaining
accuracy and precision.
The process involved carefully examining each document, scanning it with utmost care,
and transcribing the pertinent information onto separate sheets of high-quality bond paper.
Ensuring the proper arrangement and security of the records, I diligently bound them together
using a puncher and fastener. This meticulous process demanded unwavering focus and an
unwavering commitment to excellence. Despite the complexities and challenges that emerged
along the way, I remained resolute, dedicating three weeks of concerted effort to successfully
accomplish the project.
Through this transformative experience, I gained a profound appreciation for the
importance of maintaining an organized and efficient record-keeping system. The task further
solidified my understanding of the indispensable role played by meticulous attention to detail
and unwavering commitment to accuracy in administrative processes. Furthermore, the project

Throughout the course of this week, I dedicated my time to the ongoing task of
meticulously documenting the inventory of files for endorsement. As the days progressed, I
found myself faced with mounting challenges and the arduous nature of the task became
increasingly apparent. In order to combat the feelings of frustration and maintain a productive
mindset, I devised a strategy to introduce variety into my work routine. To break the monotony
and provide myself with a mental reprieve, I took on additional tasks alongside the inventory
writing. One such task involved making photocopies of various documents, ensuring the
replication of important records for safekeeping. By engaging in this secondary activity, I not
only refreshed my mind but also contributed to the efficient organization and preservation of
vital information.
In addition, I dedicated time to filing employer records in their designated
municipalities, a task that required careful attention to detail and adherence to specific
procedures. By meticulously organizing and categorizing these records, I not only contributed
to the overall efficiency of the workplace but also gained a sense of accomplishment and
satisfaction.
Furthermore, I took on the responsibility of delivering various documents within the
MSS. This involved navigating through different departments or locations, ensuring the
seamless transfer of essential paperwork. The act of physically delivering these documents not
only allowed me to step away from my primary task temporarily but also provided an
opportunity to engage with colleagues and build professional relationships.
Amidst the array of tasks I undertook this week, I also received news that added a layer
of emotional significance. I learned that my immediate supervisor, Ma'am Jan, would be
departing from our team on March 30. This revelation stirred a mixture of emotions among us,
as Ma'am Jan is not only a valuable member of our professional network but also someone who
holds a close and cherished place within our personal lives. The impending departure of such
a trusted and respected individual presents a significant shift in our dynamics and creates a
sense of loss within our team.
As I navigate through the challenges of inventory writing and the complexities of
workplace dynamics, I recognize the importance of finding coping mechanisms and
maintaining a positive outlook. By diversifying my tasks, engaging in meaningful interactions,
and actively addressing my frustrations, I strive to uphold productivity while simultaneously

During the course of the week, a fortunate turn of events unfolded as my immediate
supervisor, Ma'am Jan, took a leave of absence. Consequently, my workload in the office
became comparatively lighter than usual. Seizing this opportunity, I enthusiastically decided
to maximize my time and actively contribute to the team's objectives. With a proactive mindset,
I approached Ma’am Cee Cee, who happens to oversee the work of my colleague, Anthony,
and offered my assistance, recognizing the potential to make a meaningful contribution to our
collective efforts.
Gratefully, Ma’am Cee Cee entrusted me with a task that involved calculating the
overtime hours for employees belonging to a specific company. As I immersed myself in this
assignment, a meticulous examination of the provided formula revealed several errors. Driven
by my determination to overcome this challenge, I took it upon myself to develop an improved
solution. Methodically analyzing the data, I devised a revised formula that rectified the
inaccuracies and ensured precise calculations. This diligent effort enabled me to successfully
complete the task within the allotted timeframe, effectively showcasing my problem-solving
abilities and unwavering commitment to delivering work of the highest quality.
Furthermore, I proactively reached out to Ma'am Michelle during the same week,
recognizing the workload burden on Tricia and Anthony. With genuine enthusiasm and a desire
to alleviate their responsibilities, I expressed my willingness to support them in the crucial task
of verifying employer records. Both Ma'am Michelle and my colleagues warmly welcomed my
offer, appreciating the assistance I could provide. Embracing the responsibility with a strong

relationships with coworkers and bosses can also take a toll on both my mental and physical

 I have encountered an error or discrepancy in the formula provided by Ma'am Cee Cee
in computing the OT hours of the employees from a certain company.

Solutions:
 I took the initiative to break down her formula in search of the error, and through careful
analysis, I was able to fix it. I carefully analyzed and deconstructed her formula to
identify the error, which I successfully resolved. After thoroughly examining her
formula, I was able to pinpoint and rectify the error, restoring its accuracy.
 In addition to fixing the error in her formula, I went a step further and developed my
own formula that surpasses the efficiency of her current one. Not only did I identify
and fix the error in her formula, but I also devised my own formula that offers enhanced
efficiency compared to the one she currently utilizes.

Throughout this eventful week, a significant portion of my time was dedicated to crucial
tasks that played a vital role in maintaining the efficiency and accuracy of our workplace
operations. The meticulous processing of payrolls demanded my undivided attention as I
ensured that each employee's compensation was accurately calculated and documented.
Additionally, me and my colleagues entrusted with the responsibility of generating essential
forms, including the R1, R1A, Payment Slip, Loan Slip, and various others. By meticulously
attending to these administrative tasks, I believe I contributed to the smooth functioning of our
organization's financial processes.
In addition to my payroll duties, I undertook the important task of organizing and filing
employer records according to their designated municipalities. By diligently categorizing and
arranging these records, I helped streamline our data management systems, ensuring easy
retrieval and efficient record-keeping practices.
A significant focus of my week revolved around verifying the latest payments received
from a diverse range of employers. This involved meticulous scrutiny of financial transactions,
cross-referencing data, and making necessary updates to the records. By diligently performing
these verification tasks, I contributed to the accuracy and integrity of our financial records,
fostering trust and transparency in our interactions with employers.
However, amidst the professional responsibilities, the news of Ma'am Jan's impending
departure on March 30, 2023, brought about a mix of emotions within the workplace. As a
beloved figure and respected supervisor, her departure marked the end of an era. It felt as if a
nurturing presence was being taken away, leaving a void in our daily interactions. The
experience served as a reminder of the inevitable changes and transitions that can occur in the
professional realm. While it was challenging to bid farewell to someone who had played a
significant role in our professional and personal lives, it provided an opportunity for personal
growth, resilience, and adaptation to new dynamics.
Navigating the departure of a valued colleague requires a delicate balance of embracing
the memories and experiences shared while embracing the prospect of new beginnings. It calls

 In the Accounts Management Sections, I approached each staff member and inquired if
there was any way I could assist them, considering the absence of an immediate

 I sought guidance from my colleagues, requesting them to teach me the process of
generating PRN. Through their support, I gained a basic understanding and began
practicing by generating PRNs for a few documents.

Before our 8th week of work began, me and my colleagues decided to request for
overtime on Sunday, April 2, 2023. This day was dedicated to a range of important tasks,
starting with the meticulous preparation of furniture and equipment for disposal. We ensured
that each item was properly organized, accounted for, and ready for the next appropriate step.
In addition to preparing for disposal, we conducted a comprehensive inventory of
supplies and equipment. This involved carefully documenting and verifying the quantities of
each item, ensuring accurate records for future reference. By meticulously tracking our
resources, we aimed to optimize efficiency and maintain a well-organized work environment.
During our routine lunch break within the firm's premises, something unique and
remarkable occurred. Sharing a mealtime with my colleagues provided an excellent
opportunity for us to forge deeper connections. Engaging in casual conversations, we
strengthened our bonds, nurtured a sense of camaraderie, and developed a better understanding
of one another. It was heartwarming to witness the diverse personalities within our team
coming together in a harmonious and supportive environment.
As the much-anticipated Holy Week approached, Sir Richard displayed exceptional
thoughtfulness and consideration for our well-being. Recognizing the challenges that arise
during this time, with many individuals visiting their provinces, he graciously granted us the
opportunity to leave early. This kind gesture not only saved us from potential logistical hassles
but also allowed us to better manage our time and prepare for the upcoming event. Sir Richard's
consideration and empathy fostered a positive work culture that prioritized the welfare and
work-life balance of the team.
One of the most fulfilling aspects of my work experience was the chance to visit the
PAD Section. Engaging directly with clients was a highlight for me, as it presented an
opportunity to make a tangible difference in their lives. Assisting them with their needs and,
on occasion, listening to their stories filled me with a deep sense of satisfaction. These
interactions not only honed my communication skills but also enriched my understanding of

My experience as a Customer Service Representative in the PAD department has been

expanded my knowledge in the field. This experience taught me the significance of effective
communication, empathy, and problem-solving skills when dealing with clients. Assisting

into the task at hand. Despite occasional lapses in remembering to bring my laptop, I was
incredibly fortunate to have the unwavering support of the company's vehicle, which swiftly
transported me to and from my home to retrieve the essential equipment. This seemingly small
act of assistance made me feel valued and cherished as an employee, reinforcing the sense of
belonging within the team.
Throughout three consecutive days and late nights dedicated to the intensive design
process, my reliability and ability to perform under pressure were put to the test. It was during

In summary, my experience as a student in a flexible office has underscored the

Problems Encountered:
 The design proposal presented by their colleagues did not meet the approval of Ma'am
Michelle and Sir Richard.
 Due to the influx of numerous clients visiting the office, the PAD section required an
additional trainee to accommodate the crowd.

Solutions:
 At the request of Sir Richard and Ma'am Michelle, I was assigned the task of designing
t-shirts with a special emphasis on incorporating Abel/Inabel patterns. These patterns
are derived from the traditional woven fabric known as Abel, which originates from
Vigan and the Ilocos region. The Abel cloth is renowned for its strength and vibrant
colors, often treasured as heirlooms that can endure for generations alongside antique
furnishings. By integrating the Abel patterns into the t-shirt designs as per Sir Richard
and Ma'am Michelle's request, we aimed to showcase the rich cultural heritage and
artistry associated with this traditional fabric. The incorporation of these unique
patterns not only added visual appeal but also served as a testament to the skill and
craftsmanship of the weavers.
 After the process of making revisions to the design, I was able to assist Ma'am Marydell
in effectively addressing the mounting influx of clients.

Throughout the course of this week, my primary responsibility encompassed
meticulously verifying the final payments received from various employers. This crucial task
involved conducting thorough reviews and cross-referencing payment records to ensure utmost
accuracy and completeness. Additionally, I leveraged the Smart Messaging Suite as a powerful
tool for promptly communicating with these employers via personalized text blasts, thereby
delivering pertinent updates and essential information.
The process of meticulously verifying the last payments was undertaken with utmost
diligence, aiming to ensure the utmost precision and integrity of our financial transactions. This
necessitated a systematic approach, with meticulous attention to detail, in order to mitigate the
possibility of any errors or omissions. By exercising utmost care and precision, I endeavored
to uphold the reliability and accuracy of our payment records, ensuring the highest standards
of accountability.
Simultaneously, the utilization of the Smart Messaging Suite proved to be an invaluable
asset in facilitating streamlined and efficient communication with our employers. This
sophisticated platform enabled the dispatch of tailored text messages, enabling us to furnish
the employers with crucial updates and pertinent information concerning their payments. By
harnessing this technology-driven communication channel, we fostered transparency,
engendered trust, and forged robust relationships with our employers.
The confluence of diligently verifying payments and harnessing the capabilities of the
Smart Messaging Suite bolstered the overall operational efficiency of our organization. By
methodically executing these tasks, I played a pivotal role in preempting potential delays or
issues that may have arisen due to inaccuracies in payment records. Furthermore, the adept

This week weighed heavily on my shoulders, as I found myself grappling with a
multitude of emotions that left me feeling demotivated and burdened. The void in my heart,
which I first sensed upon arriving in Ilocos, continued to grow subtly, permeating my thoughts
and affecting my overall well-being. The toll it took on my mental state was evident in my
disrupted sleep patterns, as restlessness plagued my nights and spilled over into my days,
leading to a divided attention and a nonchalant approach to my work in the office. The lack of
focus and diminished concentration further compounded the challenges I faced.
Each morning, as I awoke, I found myself engulfed in a sea of uncertainty, lacking a
clear sense of purpose or direction. It was an unfamiliar feeling, one that left me somewhat

Problems Encountered:
 Time was running out as we verify employer records.
 The constant strain of staring at the monitor for extended periods took a toll on our
eyes.
 Throughout the week, my emotional state hit rock bottom, making it incredibly
challenging to maintain focus even on the simplest tasks.
 I found myself unable to concentrate, despite engaging in activities that would typically
require minimal effort.

Solutions:
 In order to meet the deadline, we adopted a collaborative approach by distributing the
files among the team for verification, which enabled us to successfully complete the
task within the designated timeframe.
 As for my mental state, I recognized and accepted the fact that experiencing an
emotional breakdown is a natural part of the human experience. Instead of dwelling on
negative thoughts, I consciously chose to divert my attention towards more positive and
uplifting distractions. To my surprise, this conscious effort allowed me to break free
from the grip of negativity and rise above the challenges that threatened to consume
me.

Throughout the course of the week, my primary focus was dedicated to meticulously
scanning and organizing each drawer within Ma'am Cee Cee's file cabinets. The objective was
to locate any misplaced employer records and ensure they were returned to their appropriate
municipalities. This task demanded unwavering attention to detail as I painstakingly sorted
through numerous documents, meticulously verifying their accuracy and rightful placement.
For the initial three days, I devoted my efforts to rectifying any discrepancies and restoring
order to the filing system.
In the midst of my regular duties, an unexpected telephone call interrupted the flow of
my day. With no other staff available to answer it, I took the initiative to handle the call myself.
Little did I anticipate the challenges that awaited me. The caller, clearly frustrated, allowed no
room for formalities or introductions. Despite the initial turbulence, I remained composed and
maintained a professional and empathetic approach. This experience served as a profound
lesson, teaching me the importance of active listening, understanding, and demonstrating
empathy towards clients, even in the face of their frustrations.
As the week progressed, I successfully completed the task of filing the previously
misplaced employer records in their respective municipalities. Witnessing the genuine relief
and appreciation expressed by Ma'am Cee Cee upon seeing the successful culmination of this
significant assignment was incredibly fulfilling. It instilled within me a sense of pride and
accomplishment, further motivating me to strive for excellence in my work. This positive
experience served as a catalyst for renewed dedication and a desire to continually improve and
deliver exceptional performance.
With the satisfaction of a job well done, I reflected on the value of organization and
attention to detail in maintaining an efficient and effective workplace. The meticulousness

contributed to the overall productivity and smooth operation of the office. It reaffirmed my

Experience Acquired:
As an OJT student, I have come to realize that working for long hours can have
significant long-term side effects on both my physical and mental health. The sedentary nature
of desk work has taken a toll on my body, leading to back pain and poor posture. Additionally,
the constant exposure to computer screens has caused eye strain and frequent headaches,
affecting my overall well-being.
Moreover, the stress and pressure of meeting deadlines and fulfilling job
responsibilities have had a profound impact on my mental health. The constant demands and
high expectations have contributed to feelings of anxiety and burnout, making it challenging
to maintain a healthy work-life balance. Neglecting self-care and working excessive hours have
not only affected my performance at work but have also strained my personal relationships and
overall quality of life. It has become clear to me that while being an OJT student provides
valuable learning opportunities, it is crucial to acknowledge and address these potential side
effects in order to prioritize my health and well-being in the long run. Through firsthand
experience, I have realized the importance of self-care and finding a balance between work and
personal life to sustain a healthy and fulfilling career.

Problems Encountered:
 Sometimes, when our supervisors request us to retrieve specific employer records, they
are missing, leading to time-consuming searches across different municipalities.
 Multiple folders containing the same employer records are scattered across different
drawers, causing confusion and inefficiency.
 Some of the employer records have incorrect address details, which were encoded by
their respective Account Officer. These inaccuracies need to be edited and reprinted to
prevent complications and ensure accurate routing to the correct municipalities.
 The presence of misplaced or mislabeled employer records adds unnecessary hassle and
can result in delays or errors in processing.

Solutions:
I thoroughly scanned every drawer under the supervision of Ma'am Cee Cee to locate
any misplaced employer records. To my surprise, I discovered more than 50 employer records
that were erroneously filed in incorrect municipalities. Additionally, I encountered several
instances where duplicates of the same employer records were present.

This week proved to be incredibly demanding as I delved into a variety of tasks that
required both creative flair and meticulousness. My primary focus centered on design
endeavors, as I was entrusted with the responsibility of crafting three unique business card
designs for the SSS staff and three distinct designs for email signatures. The request came
directly from Ma'am Michelle, and I eagerly embraced the opportunity to showcase my design
skills and fulfill her vision.
With unwavering enthusiasm and dedication, I embarked on the design journey,
carefully considering the preferences and specifications outlined by Ma'am Michelle. Each
business card design needed to exude professionalism while capturing the essence of the
respective staff members. Through iterative revisions and collaborative feedback, I diligently
worked to breathe life into their visions, ensuring that the final designs were not only visually
captivating but also aligned with their professional identities.
As the week progressed, my focus shifted to the transfer of employer accounts to their
designated Account Officers through the AmaS system. This reassignment became imperative
due to the recent departure of Ma'am Jan, leading to Ma'am Ronil temporarily taking charge of
her profile. To facilitate a seamless transition, we meticulously reviewed and organized each
employer record, verifying the accuracy of the information and allocating them to the
appropriate Account Officer. This intricate process demanded meticulous attention to detail, as
any inaccuracies could have far-reaching consequences for the employers and the overall
efficiency of the system.
The transfer of employer accounts necessitated close collaboration among various
stakeholders. Effective communication played a pivotal role in ensuring a smooth handover
and minimizing any disruption to the employers' service. By fostering open lines of
communication and actively engaging with the Account Officers, we successfully facilitated a
seamless transition, guaranteeing that the employers' needs were met, and their accounts were
managed without complications.
Throughout the week, I encountered numerous challenges that called for technical
prowess, adaptability, and problem-solving skills. From handling design requests to executing
the transfer of employer accounts, I navigated through diverse tasks with unwavering
determination and an unwavering commitment to excellence. These experiences further
solidified my ability to tackle heavy workloads and devise effective strategies to overcome
obstacles. I gained invaluable insights into the significance of attention to detail, effective

Problems Encountered:
 The records in the drawer of Laoag MUB were positioned in the wrong direction,
making it difficult for us to locate and navigate through them efficiently.
 The drawer of Laoag, managed by Ma'am Fatima, was excessively packed, making it
challenging for us to retrieve and file individual documents due to the tightness and
limited space within the drawer.

Solutions:
To improve the organization and accessibility of the Laoag MUB drawer, we took the
initiative to flip the records in the drawer to the right direction. This simple adjustment made
it much easier for us to navigate through the documents efficiently. In addition to repositioning
the records, we also assessed the contents of the drawer and identified the need for decluttering.
By carefully removing some of the records that were no longer needed or could be relocated
elsewhere, we created more space and alleviated the tightness that made it challenging to pull
out and file individual documents. Through our efforts, we aimed to enhance the functionality
of the drawer, ensuring that it could accommodate the records effectively while facilitating
smoother retrieval and storage processes.

This week marked a significant milestone in my internship journey as I was formally
introduced to Ma'am Cee Cee, my official immediate supervisor. Right from our first meeting,
I could sense that she possessed a remarkable depth of knowledge and experience, coupled
with a warm and supportive demeanor. Ma'am Cee Cee has been an incredible companion and
mentor, offering valuable guidance and insights that have shaped my understanding of the job.
Her professionalism and unwavering dedication to her work have been truly inspiring, and I
consider myself fortunate to have her as my supervisor.
In addition to meeting Ma'am Cee Cee, I had the privilege of participating in a field
visit led by Sir Richard, alongside Ma'am Michelle and Sir Bong. Our itinerary included visits
to the Mariano Marcos Memorial Hospital and Medical Center, as well as the office of the
municipal mayor in Paoay, Ilocos Norte. The purpose of the visit was to conduct follow-ups
on the SSS accounts of their trainees. This hands-on experience provided me with a tangible
understanding of how our work translates into real-world applications and highlighted the

Problems Encountered:
During this week, the PCs in our office were painfully slow, causing significant delays
in our daily tasks and hampering productivity. It was clear that something needed to be done
to address this issue. After careful evaluation, we realized that one of the main factors
contributing to the sluggish performance was the outdated version of Windows 10 installed on
the machines. It became evident that upgrading to a better version of Windows 10 was
necessary to enhance the overall speed and functionality of our office computers.
Upgrading the PCs to a newer version of Windows 10 brought about a remarkable
improvement in their performance. The updated operating system provided various
optimizations and enhancements that significantly boosted the speed and responsiveness of the
machines. With the upgraded version, the PCs were able to handle multiple tasks seamlessly,
reducing the frustrating lags and delays we had experienced before. The improved efficiency
translated into enhanced productivity, allowing our team to accomplish tasks more swiftly and
efficiently. Upgrading the PCs to a better version of Windows 10 proved to be a vital step in
ensuring a smooth and efficient workflow in our office.

Solutions:
Sir Richard recognized the urgent need to address the slow performance of the office’s
PCs took the initiative to appoint two technicians from Manila to assist with the upgrade. These
technicians tasked us with the responsibility of backing up all the files from the computers
before proceeding with the system upgrade. The backup process was essential to ensure that
no valuable data would be lost during the upgrade, as upgrading systems can sometimes result
in the deletion of certain contents.
Over the course of several days, the technicians diligently worked on upgrading the
PCs, striving to improve their performance and functionality. However, despite their efforts,
the outcome was not as anticipated. Once the upgrade was completed, we found that there was
not a significant difference in the performance of the PCs. To make matters worse, we
encountered errors when attempting to connect to the SSS VPN server, which became a major
inconvenience for our team. The initial hope for a smoother workflow and enhanced
productivity seemed to have been overshadowed by these unforeseen challenges.
Despite the frustrations caused by the upgrade, it is important to acknowledge the
efforts made to address the performance issues of the office PCs. While the expected
improvements may not have been fully realized, it is possible that the technicians encountered
unforeseen complexities during the upgrade process. It may be necessary to reassess the
situation, communicate the challenges faced with the technicians, and explore potential
solutions to overcome the errors in connecting to the SSS VPN server. By working closely with
the technicians and seeking their expertise, it is possible to resolve the issues and ultimately
achieve the desired performance and functionality for our office PCs.

mayor's reception was characterized by a graciousness that left a deep impression on us.

Throughout this week, my mind was consumed by contemplation about the inevitable
day when we would bid our final goodbyes to the office that had become a second home to us.
I invested considerable time and effort in building genuine and meaningful relationships with
the people around me, especially within the AMS department. Now, as the time to depart
approached, it felt as though we were leaving behind not just a workplace but a place filled
with memories that we had shared—a place that housed a supportive and nurturing family who
had played a vital role in my personal and professional journey. The realization of these
emotions served as a poignant reminder of one of the inherent consequences of being an
employee, where the connections we forge become an integral part of our lives and leave lasting
imprints on our hearts.
Amidst the swirling emotions, I dedicated the remainder of the week to the PAD
Section, immersing myself in interactions with clients and providing assistance with their
respective transactions. It was a gratifying experience to be able to offer support and contribute
to the seamless functioning of the office until the very end. Engaging with the clients allowed
me to witness firsthand the impact of our work and the positive difference we had made in their
lives. Their expressions of gratitude and appreciation served as a testament to the importance
of our collective efforts as employees, and it reinforced the significance of the role we played

As our last day loomed before us, I eagerly offered my assistance to the Admin Office,
aiding in the preparations for the resignation march of one of their esteemed employees. This

their time and skills to the betterment of the office. It was a solemn yet meaningful event that
symbolized the passing of the torch and acknowledged the contributions of those who had come

During my fulfilling three-month tenure at the SSS Laoag Branch, I had the privilege
of working in the Accounts Management Section (AMS), where I immersed myself in a
multitude of processes and functionalities that contribute to the seamless operation of the
office. This invaluable experience provided me with a comprehensive understanding of the
inner workings of the department and allowed me to develop a diverse skill set.
One of the fundamental aspects of my role in the AMS was the payroll management
process. I acquired in-depth knowledge on how to meticulously compute and process employee
salaries, ensuring accuracy and adherence to established protocols. By learning this critical
task, I gained insight into the complexities and meticulousness required in handling financial
transactions within the organization. Another essential responsibility I undertook was verifying
the SSS numbers of clients. This involved cross-referencing records, meticulously validating
information, and ensuring that accurate data was maintained in the system. This task not only
sharpened my attention to detail but also reinforced the importance of maintaining data
integrity and confidentiality.
In addition, I had the opportunity to engage with employers through text blasts
regarding their last payments on their contributions. This involved effectively communicating
payment reminders and ensuring timely follow-ups. Through this experience, I honed my
communication skills and learned to handle various queries and concerns with professionalism
and efficiency. Interacting with a diverse range of clients was an enriching aspect of my time
in the AMS. I engaged in face-to-face interactions, addressing their inquiries, providing
guidance, and facilitating the completion of necessary documentation. This direct client
engagement helped me develop strong interpersonal skills, patience, and the ability to handle

GENERALIZATION

SUMMARY
Conducting my OJT far away from home was an incredibly transformative
experience, especially for someone like me, an introvert who typically finds comfort in
familiar surroundings. It marked a pivotal moment in my personal growth as I summoned the
courage to step out of my comfort zone and embrace the unknown. As I ventured into the city
of Laoag, the scorching heat welcomed me, but little did I know that it would become a
formidable adversary, causing me to fall ill for a few days. Nevertheless, I viewed this
setback as an opportunity to build resilience and adaptability, traits that would serve me well
throughout my journey.
The weather and overall atmosphere in Laoag mirrored that of my hometown,
Pamplona, but it possessed an intensified quality. Every morning, as I prepared to leave for
work, I faced the daunting challenge of preventing beads of sweat from forming on my
forehead, a battle I had yet to conquer. The relentless heat became a daily reminder of the
physical and mental fortitude required to thrive in unfamiliar surroundings.
Arriving at the SSS Laoag Branch on my first day, I was immediately struck by the
stark contrast in office culture and dynamics. It was a culture shock that jolted me out of my
comfort zone, forcing me to adapt quickly. The early morning wake-up calls and the exhaustion
that settled in each evening demanded a disciplined routine, altering my sleep patterns and
reinforcing the importance of self-care and time management.
Throughout my internship, the most invaluable lesson I learned was the power of
effective communication. As an introvert, I initially found it challenging to connect with others
and express myself openly. However, I recognized that building strong relationships with my
supervisors and peers was not only vital for personal growth but also instrumental in achieving
the company's goals. I discovered the art of active listening, fostering an environment where

Upon entering the office, my attention was immediately drawn to the impeccable
cleanliness, formal atmosphere, and professional demeanor exhibited by the staff. Their
commitment to maintaining an organized and pristine workplace was evident. Moreover, the
overwhelming generosity and kindness displayed by nearly every employee quickly dispelled
any initial anxieties I had, making me feel welcomed and at ease.
During my tenure, I did encounter one aspect that left me feeling somewhat uneasy. It
became apparent that there was a clear division in relationships between the staff on the second
floor and those on the ground floor. Although I couldn't ascertain the exact reasons behind this
separation, it was evident that each group had established their own personal space and
interactions between the floors were limited. Respecting their boundaries, I refrained from
delving further into the matter, understanding that personal relationships can develop
differently within different workplace dynamics.
As I delved deeper into my internship, I faced certain challenges within the office
environment. Notably, the office building itself was insufficient in size to comfortably
accommodate the growing number of clients and employees. The limited space created a sense
of congestion, particularly during peak hours, hindering smooth movement and efficient
workflow. The constraints imposed by the cramped conditions served as a constant reminder
of the pressing need for a more spacious and accommodating workplace.
Another issue that caught my attention was the flooring on the second floor, specifically
in the conference room, which consisted solely of wood. This choice of flooring material posed
potential risks when large numbers of people gathered in the room. The instability of the
wooden flooring raised safety concerns, as it could lead to accidents if not properly maintained.
It became evident that a more secure and sturdy flooring solution should be considered to
ensure the well-being of both employees and clients.
In addition to the physical infrastructure, I also observed limitations in the office's
technological systems. The Windows operating system processors were notably sluggish,
impeding the smooth processing of documents and causing delays in daily tasks. Upgrading to
a more advanced version of Windows 10 or exploring alternative hardware options could
significantly enhance efficiency and productivity. Furthermore, the office's reliance on
OpenOffice, instead of having a Microsoft Office subscription, presented certain challenges.
While OpenOffice served its purpose, it offered limited functionalities and features, posing
difficulties, particularly for newcomers like myself who were accustomed to the commonly
used features and interfaces of Microsoft Office.
