Private Hire Driver Licence Application

You must confirm each of the following


statements before continuing to make an
application:
You are over 18 years of age and have held
a valid full UK driving licence for at least 12
months
You must have a valid drivers licence to continue this
application.

You have an acceptable image of your


driving licence
– A clear image of the front and back of your
driving licence is required. See example

You must have a suitable image of your licence to


continue this application.

You have an acceptable drivers photo


– The Passport type photograph must include
your head & shoulders and be taken with a
white/pale background. See example

– This can be taken by yourself on a digital


camera/smartphone and uploaded as part of the
application process.

You must have a suitable photo of yourself to


continue this application.

You have a valid Medical Certificate proving


you are fit to DVLA group 2 standard
– To obtain a medical certificate from a doctor,
please ask them to complete and sign the form
which can be downloaded here. You must use
this form for all medical assessments attended
after 01/09/2016.

You must have a valid medical certificate.

You have the legal right to work in the UK


– You must have the correct immigration status to
apply for a private hire drivers licence, and the
licence will lapse if you cease to be entitled to
work in the UK. Your right to work in the UK will
be checked as part of your licensing application.
You will be required to bring original documents,
such as a passport or biometric residence permit,
so that the check can take place. You will be
advised further in this matter before a licence is
issued.

You must be eligible to work in the UK to continue


this application.

I am subscribed to the DBS Update Service


or I am willing to provide an Enhanced DBS
Certificate every six months.
– From 1 April 2021, City of Wolverhampton
Council will require all applicants providing a DBS
certificate to subscribe to the DBS Update
Service. The DBS certificate must be for an
enhanced check for the position of ‘Other
workforce taxi driver’ or ‘Child and adult
workforce Taxi driver/ PHV driver’. We will
not accept certificates for ‘adult workforce Taxi
driver/ PHV driver’. If you do not subscribe to the
Update Service and your licence is granted or
renewed after 1 April 2021, you will be required to
purchase a new DBS certificate every six months.
Six monthly DBS checks are now required by the
Department for Transport.

You must confirm you will provide Enhanced DBS


certificates every six months

I am aware of my tax responsibilities.


– From 4 April 2022 you have responsibility to
provide a HMRC tax check code in certain
circumstances e.g. you have held a licence with
another Licence Authority or have held the same
type of licence that ceased to be valid less than a
year ago. Please refer to GOV.UK website
addresses to view the HMRC guidance about tax
registration obligations and clarify your position:

– Registering for Self Assessment

– Corporation Tax Information

– Confirm Your Tax Responsibilities

You must confirm that you have completed the tax


check.

Important information
HMRC has powers to obtain information from
Licensing Authorities using ‘Schedule 23 to
Finance Act 2011 (Data Gathering Powers) and
Schedule 36 to Finance Act 2008 (Information
and Inspection Powers), grant HMRC powers to
obtain relevant information from third parties.
This includes licensing bodies being required to
provide information about licence applicants.’

The application fee for a New Private Hire


Driver licence is £49.00 for a 1 year licence or
£98.00 for a 3 year licence and must be paid
electronically as part of this application process.
You therefore need a debit or credit card to
make the payment..
Application fees will not be refunded

You must also complete a DBS application


(Criminal Records Check) and provide a DVLA
three year mandate consent both of which must
be completed with Personnel Checks.
These can be completed before you apply
and you upload the result as part of this
application or you can apply after
submitting this application. Your licence
will not be provided without completion of
this check.

In order to obtain a Private Hire Drivers Licence


you must have successfully completed our
Private Hire Training & Assessment
programme. This can be completed before or
after you make this application but a licence will
not be issued until successful completion.

place on the course and make payment.

The licensing authority provides information to


the National Register of Taxi Licence Refusals,
Revocations and Suspensions (NR3S), a
mechanism for licensing authorities to share
details of individuals who have had a hackney
carriage or Private Hire Vehicle (PHV) licence
suspended, revoked, or an application for one
refused. This is necessary for the performance
of a task carried out in the public interest or in
the exercise of official authority vested in the
licensing authority – that is, assessing whether
an individual is a fit and proper person to hold a
hackney carriage or PHV licence Therefore:
Where a hackney carriage/ PHV licence is
suspended, revoked, or an application for
one refused, the authority will
automatically record this decision on
NR3S.

All applications for a new licence or


licence renewal will automatically be
checked on NR3S. If a search of NR3S
indicates a match with an applicant, the
authority will seek further information
about the entry on the register from the
authority which recorded it. Any
information received as a result of an
NR3S search will only be used in respect
of the specific licence application and will
not be retained beyond the determination
of that application.

The information recorded on NR3S itself will be


limited to:
name

date of birth

address and contact details

national insurance number

driving licence number

decision taken

date of decision

date decision effective

date of any subsequent change to the


decision, and the date it takes effect, if
different

if a suspension, the date on which the


suspension is to end

Information will be retained on NR3S for a


period of 11 years.

This is a mandatory part of [applying for] [being


granted], a hackney carriage / PHV driver
licence. The authority has a published policy on
the approach it will take to requests by other
authorities for further information about entries
on NR3S, and about the use it will make of any
further information provided to it. Information
will be processed in accordance with the Data
Protection Act (DPA) and General Data
Protection Regulation (GDPR). Any searches,
provision or receipt of information of or under
NR3S are necessary to the authority’s statutory
licensing functions of ensuring that all drivers
are fit and proper to hold the applicable licence.
It is not intended that any NR3S data will be
transferred out of the United Kingdom. If you
wish to raise any issue related to the data
protection legislation, including by relying on
any of the rights afforded to data subjects under
the GDPR, you can do so to the authority’s
Data Protection Officer. This includes
submitting a subject access request.

You always have the right to make a complaint


to the Information Commissioner’s Office (ICO).
