    AFFIDAVIT OF PAYMENT
    (WITH ACKNOWLEDGEMENT OF RECEIPT OF PAYMENT)

    I, JERRY A. FANO of legal age, single/ married , with address at 73


    Linaluz St., San Carlos, Binangonan, Rizal , referred to as the BORROWER, after
    being sworn in accordance with law, depose and say that:

    1. In November 2021, due to the COVID-19 pandemic my personal finances was hit hard
    and I borrowed money from Mr. EMERLINO MANAID of
    legal age, single/ married , with address at Bangad, Cabanatuan City , referred
    to as the LENDER, in the total amount of Php 1,000,000.00 .
    2. Due to humanitarian reason and in order to extend financial assistance, the LENDER
    willfully agreed to provide loan to the BORROWER in the total the amount of
    Php 1,000,000.00 - with no interest – provided that the BORROWER will
    pay the LENDER in installment basis the full amount payable within two years or period
    from November 2021 to November 2023.

    3. This affidavit is executed as a proof of good faith on the part of the BORROWER in
    making good his obligation to pay under the agreement the full amount (with no
    interest) based on the following paid installment(s) made, to wit:
