MEMORANDUM OF AGREEMENT
KNOW ALL MEN BY THESE PRESENTS:
This Agreement made and entered into this 21 st day of September, 2023 by and
between:
The TULA - TULA PEQUEÑO ELEMENTARY SCHOOL of Ligao City Division with
School Address at Tula - Tula Pequeño Ligao City
And
The CONVOY OF HOPE PHILIPPINES, Inc,. with office address at 4th and 5th floor, New
Form Realty Building, 1924 Taft Avenue Cor, Bernabe St., Pasay City 1300 Metro Manila. Represented by herein
Requesting Staff, RISHELLE DICHOCO - ANTONIO referred to as DONOR.
TULA - TULA PEQUEÑO ELEMENTARY SCHOOL and DONOR shall be collectively called
"Parties"
WITNESSETH
Whereas , the Department of Education (DepEd), through the Bureau of Learner Support
Service - School Health Division (BLSS - SHD), is mandated to implement a nationwide a School-
Based Feeding Program (SBFP) Pursuant to the mandates and objectives of RA No. 111037 or the
"Masustansiyang Pagkain para sa Batang Pilipino Act" and the General Appropriations Act (GAA)
for the Fiscal Year 2023 - 2024;
Whereas, R.A. No. 11307 provides the inclusion of Fortified Rise Against Hunger (Manna
Pack) product in the SBFP as additional component
Whereas, the Tula Tula Pequeño elementary School and the Convoy of Hope Philippines
Inc,. (COHP) have agreed to embark on and implement a Fortified Rise Against Hunger (Manna
Pack) feeding program "(Program hereafter)"pursuant to the mandates and objectives of R.A. No.
11307 and as a component of the National Feeding Program of the government;
Whereas, the First Party has the capacity to provide the full support in the distribution of
the food feeding program to the learners from kinder to grade 6 and hereby warrants its
improvement of SW and W learners from baseline to endline.
NOW THEREFORE. For and in consideration of the foregoing premises and the mutual
covenants stipulations and agreements hereof, the parties do hereby agree, and contact as
follows:
Article 1
RESPONSIBILITY OF FIRST PARTY
1. Prepare the list of learners of the recipients per grade level.

2. Plan organize and facilitate the whole program.
3. Ensure the 100% observance of the Health Safety Protocol of all participants
4. The agreement shall govern the operative undertaking between the parties in the
implementation of
the program.
5. The parties agreed to cooperate and carry out their respective roles and responsibilities
stated in this
Agreement and in the Framework MOA and comply with the guidelines executed and
issued for this
Program
IN WITNESS WHEREOF, the Parties hereto have caused this Agreement to be executed in
accordance with governing laws on the day and year first above written.
