TRANSPORTATION OF MEN & MATERIAL USING JCB /


HYDRA / CRANE – IT CAN BE FATAL
In one of the refineries, one JCB was found transporting men as well as
materials through refinery roads to construction site. Two men were
found sitting along with driver inside the cabin. Moreover, the JCB was
transporting shuttering materials such as steel pipes, wooden panels
etc. in its bucket.
Incident Recall: Several accidents have occurred in the past in various
locations while transportation of men and materials using JCBs, Hydras,
Cranes etc.
Such unsafe acts by JCB/Hydra/Crane operators shall never be allowed
inside refinery and corrective actions must be taken to avoid any
untoward incident.

VESSELS NOT FREED FROM HAZARDOUS MATERIAL!!!


A vessel was divided into two halves by a baffle, which had to be
removed. The vessel was cleaned out, inspected and a permit was
issued for a worker to enter the left-hand side of the vessel to remove
the baffle by hot work. It was impossible to see into the right-hand side.
But because the left-hand side was clean and because no combustible
gas could be detected, it was assumed that the other side was also
clean. While the welder was performing hot job, some deposit in the
right-hand side caught fire. The welder got out without serious injury
but bruised himself while coming out of the vessel.

Things to Remember:
• If a part of the vessel cannot be inspected, then we should assume
the vessel contains hazardous materials.
• If the previous contents were flammable, we should assume there is
some flammable material out of sight.
• If the previous contents were poisonous, we should assume there is
some poisonous material out of sight, and BA Set should be worn for
entry.
• Gas tests alone are not conclusive. There may be some sludge
present that gives off gas when heated or disturbed.
