    Service Requested: - Conduct - Safety Radio Survey for endorsement of the DNV - Safety Radio Certificate. (SRC)
    Action Taken:

    Attended vessel in Luanda anchorage and conducted the following inspections as stipulated below

    1. Check radio station licence "Marshall Islands" License#:70239, -OK


    Date of issue: 15-06-2021 valid till 14-06-2025 - OK
    MMSI: 538003546, IMO: 9396309, Call sign: V7RN7. - OK
    2. SHORE BASED MAINTENANCE agreement. Exp 10-10-2025. - OK
    3. Check, decode and certify the EPIRB 1 as per attached documents. - OK.
    4. Check AIS as per attached DNV check sheets. OK.
    5. Check both SART's as per attached documents. OK.
    6. . Check all GMDSS equipment as per attached DNV check sheets. OK.

    Defects listed: Epirb HRU "Hammar H20R" found to be expired, Vessel has made a request to replace asap.
    • Recommended that Epirb be placed under "Manual activationn until HRU is replaced.

    No other defects found. The SRC may be endorsed as per the above DNV check sheets.
    All Equipment As stated above Checked and in good operational order as required by " Marshall islands" flag

    This Service Report I Time Sheet is a true reflection of the activities and work completed.
