A. DOCUMENTARY REQUIREMENTS:
1. Passport with validity period of not less than six (6) months
2. Valid Work Visa, Entry/Work Permit (whichever is applicable per country).
If visa assurance or guarantee is issued by employer, the same should be
noted/acknowledged by the Government or Immigration Office in the jobsite.
3. Employment Contract or Offer of Employment (ORIGINAL COPY or VERIFIABLE
ELECTRONIC COPY)
_____ Verified by the Migrant Workers Office (former POLO)
_____ Authenticated by the Philippine Embassy/Consulate for countries with no MWO
_____ Apostille with MWO Verification
_____ Apostille with Philippine Embassy Acknowledgment
_____ For DFA/Philippine Government Official Posted Overseas’ Private Staff –
Notarized Employment Contract
_____ For JET Recipients – Notice of Appointment with confirmation from Japan Embassy
_____ For Stagiaires Recipient – Employment contract with confirmation from SEM
(Swiss Government)
4. Additional country-specific requirements:
a. Canada- Labor Market Opinion (LMO), Labor Market Impact Assessment (LMIA) for and
Canadian Letter and Employer’s Certificate of Registration from ECON (Province of
Saskatchewan Executive Council) or Saskatchewan Immigration Nominee Program
(SINP) approval required from workers to Saskatchewan in lieu of LMO
b. USA-Labor Condition Application and Notice of Action
c. Middle East and African countries- Contingency plan issued by the employer
5. Additional documents to support job application - TESDA NC II/PRC License
6. Statement on how the worker secured his/her employment.
7. Copy of Diploma/TOR, COE and other certificate as maybe applicable.
8. MWO Endorsement letter (if applicable)
