Pallor

Definition: Pallor is a pale colour of the skin, which can be caused by illness or
anaemia. Pallor is more evident on the lips, palms, tongue, mouth, mucous
membrane and conjunctivae especially in dark skinned children

When a child presents with pallor:


• Stabilize the child
• See ABCD (page xx)
• Assess the child for shock (see shock page xx)
• Perform a complete history and physical examination (see paediatric history
and physical examination page xx)
• Test haemoglobin

Think of the following diseases:


Severe blood loss (see haemorrhagic shock page xx)
Shock (see shock page xx)
Malarial and malarial anaemia (see malaria page xx)
Pain (see pain page xx)
Malnutrition (see malnutrition page xx and see MSF Nutrition guidelines)
Nutritional Anaemia (iron Folate) (see anaemia page xx)
Childhood malignancy (leukaemia etc.) (cannot be treated in MSF fields)
Disseminated intravascular coagulation (disease associated with sepsis and shock)
HIV (see MSF HIV guidelines)
Hypoglycaemia (see page xx)
Lead poisoning chelation not available in MSF fields but it is important to try to
identify the source of contamination (see ingestions page xx)
Parasites (see anaemia page xx)
Respiratory failure (see pneumonia page xx and shock page xx)
Seizures (see seizures page xx)
Sickle Cell disease (with or without splenic sequestration) (see sickle cell page xx)
Thalassemia (see anaemia page xx)
Tuberculosis (see MSF TB guidelines)
