• Data Privacy: By voluntarily filling up this form, you agree to the collection of your personal information. The
collected personal information will be solely used to enable us to establish your identity and evaluate your
application. Rest assured that the data you will provide will be restricted to PIChE and will not be shared with
any outside parties.
• Commitment Fee: I understand that a commitment fee amounting to PhP 20,000 is needed to enter the
program, unless qualified in the scholarship. The commitment fee shall be returned at the end of the program
given that I have complied with the minimum passing requirements set by the program directors. I understand
that failure in doing so will cause a non-return of the PhP 20,000.
• Certificate of Participation vs. Achievement: I understand that even if I finish the 10-week program, I still need
to conduct a complete circular economy project (i.e. feasibility study to implementation and presentation of
results) to get a Certificate of Achievement. Otherwise, finishing the 10-week program will only award me the
Certificate of Participation.
• Return of Service Agreement: I understand that after the 10-week program program, I shall render my service
to PIChE as a technical expert on Circular Economy to train and develop future circular economy champions
for at least a year.
